Requirements

Under general supervision, this position will serve as a case manager for the Laurens County HOPWA Program.  The case manager will take referrals for housing services from the Laurens County Board of Health Specialty Clinic and other approved agencies, schedule intakes, determine eligibility based on information gathered, provide ongoing case management, and complete required documentation.  The case manager will organize and maintain client files in accordance with Georgia Department of Community Affairs regulations.   The case manager will provide referrals to community resources and assist clients with accessing these resources, as necessary, and provide nutritional and transportation assistance, if needed.   The case manager will work closely with the HOPWA Program Coordinator and collaborate with Laurens County Board of Health Specialty Clinic staff and other community partners. This position will be required to participate in training and inter-disciplinary meetings.   This position requires travel throughout our 10-county area to complete housing inspections, quarterly face-to-face visits, and annual assessments.   

Qualifications

Bachelor’s degree in a related field from an accredited college or university and three (3) years of experience in social service delivery or one (1) year of experience as the lower-level Social Worker, NonLicensed 1 (HCP120) or position equivalent.
