This position will be based in Columbus, Ohio, with extensive travel required to communities in Southern Ohio.

What You'll Do:

The Center of Public Health Excellence (CPHE) is seeking one (1) dedicated Human Services Program Consultant to support the Rural Health Transformation Program (RHTP), a statewide initiative aimed at improving health outcomes and addressing unmet health needs in Ohio’s rural communities. Job duties include:
  • Provide program consultation, technical assistance, and ongoing support to RHTP‑funded communities.
  • Review programmatic and budget reports for compliance and prepare summaries for state and federal partners.
  • Monitor funded entities through site visits, phone communication, and reporting forms to ensure eligibility and adherence to program requirements.
  • Develop and present program materials such as fact sheets, applications, guidance documents, and web content.
  • Assist communities in achieving objectives related to diabetes prevention, hypertension management, improved nutrition, and increased physical activity.
  • Plan and facilitate workshops, trainings, conferences, and technical assistance sessions based on identified community needs.
  • Assess rural community needs by evaluating outcome and funding reports, identifying trends, and connecting organizations to resources.
  •  Support grant development, contract management, invoice review, budget revisions, RFP creation, and funding recommendations.
  • Maintain program guidelines, policy manuals, required records, newsletters, and reports.
  • Respond to inquiries from healthcare professionals, program participants, government officials, and the public.
  • Utilize GMIS 2.0, WISE, Clear Impact, and other systems to analyze data and generate reports.

Normal working hours are Monday through Friday 8:00 am to 5:00 pm. This is an hourly position, with a pay range of 12 on the 1199 Pay Range Schedule.

Qualifications

Position Qualifications:

Completion of undergraduate core program in social or behavioral science, health or pre-medicine or comparable field; AND 18 mos. exp. in delivery of human services, health care services or medical assistance in governmental, community or private human support services agency or medical provider. 

-Or completion of graduate core program in social or behavioral science or medicine-related or comparable field; AND 6 mos. exp. in delivery of human services, health care services or medical assistance in governmental, community or private human support services agency or medical provider. 

-Or equivalent of Minimum Class Qualifications for Employment noted above. 
 

*Applications of those who meet the minimum qualifications will be further evaluated against the following criteria:

  • Bachelor's or advanced degree in social or behavioral sciences, medical or health related field

  • Certified Health Education Specialist

  • Experience planning and managing population health programs related to diabetes, hypertension, nutrition and/or physical activity

  • Experience providing consultation/technical assistance to health care and/or community health centers in rural areas to improve patient outcomes and workflows

  • Experience aligning programmatic strategies with intended health-behavior outcomes
  • Experience in program budgeting
  • Experience developing performance measures
  • Experience managing grants and providing technical assistance to grantees
  • Experience analyzing program evaluations and making adjustments to programs
  • Experience writing technical reports, articles, web content and/or correspondence
  • Experience preparing and delivering presentations

Unless required by legislation, the selected candidate will begin at Pay Grade 12, Step 1 of the union (SEIU/1199) Pay Range Schedule ($32.69 per hour), with an opportunity for pay increase after six months ($34.35) of satisfactory performance and then a yearly raise thereafter.
