REHABILITATION EVALUATOR


The mission of Pinecrest Supports and Services Center is to support people with intellectual and developmental disabilities to reach treatment goals and to return to more integrated community living settings. Pinecrest Supports and Services Center specializes in the treatment of people with comorbid intellectual and developmental disabilities and complex medical, behavioral, and psychiatric support needs.

Pinecrest Supports & Services Center (PSSC) is seeking a dedicated Rehabilitation Evaluator  to join our dynamic team. We are seeking out an individual that can be responsible in assessment and evaluation for placement of our clients in work environment settings conducive to their abilities, interests. levels of achievement and  goals.

Requirements

One year of experience in rehabilitation evaluation plus one of the following:

A bachelor’s degree plus one year of experience in rehabilitation services, social services, human resources, education, therapy services, or nursing; OR

A bachelor’s degree in a rehabilitation field, a social sciences field, social work, education, nursing, health and human services, or speech therapy; OR

A master’s degree.

Job Duties:

  • Independently assess and evaluate works for placement in settings conducive to their abilities, interests, levels of achievement and goals.
  • Write Profile of Support (POS) and Individual Support Plan (ISP) for all working individuals in caseload.
  • Locate jobs that match the wants and skills level of individuals we are serving to ensure individuals have the knowledge and skills to choose where they work.
  • Process information and consult with other professionals at their individual team meeting for valid information to ensure assessments are valid and functional.
  • Attend POS and ISP meetings of all working individuals in caseload.
  • Observe and document on individuals in caseload at their work site at least once a month.
  • Write objectives, strategy sheets and follow progress of individuals in caseload.
  • Assist in giving out checks and coupons on paydays.
  • Distribute and pick-up weekly timesheets for individuals.
  • Maintain accurate records in compliance with Fair Labor Standards Act, Section 14C, as well as Louisiana Department of Revenue.

Location:  Pinecrest Supports and Services Center - Employment Services

Appointment Type: This position may be filled by Probation, Promotion, Demotion, Lateral Transfer, or Detail to Special Duty.  

Schedule: Shift:  7:30 AM - 4:00 PM    Pass Days:  Saturday and Sunday

Compensation: $20.06 hourly - $36.14 hourly
