Serves at the OAAS My Choice Transition Coordinator (TC) for the My Choice LA (MCL) project. These positions are work from home positions and are domiciled to assure statewide coverage.

My Choice Louisiana (MCL) is a state program within the Louisiana Department of Health (LDH) and operated jointly by the Office of Aging and Adult Services and the Office of Behavioral Health. MCL grew out of an agreement between the LDH and the United States Department of Justice (DOJ). In this agreement. LDH committed to increasing services and supports giving individuals in Medicaid Long Term Care institutions (LA Nursing Facilities) with Serious Mental Illness (SMI) and physical limitations due to disability increased opportunities for successful transitions into communities of their choosing. 

The Transition Coordinator (TC) is responsible for coordinating and monitoring the successful transition of individuals from nursing facilities to community-based settings. The position collaborates with multidisciplinary teams, provides technical assistance, addresses barriers to transition, and supports policy development and statewide initiatives to expand community-based services and improve transition outcomes.
  

Requirements

Five years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

Six years of full-time work experience in any field plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

A bachelor’s degree plus two years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR

An advanced degree or a Juris Doctorate plus one year of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

JOB DUTIES:·  

  • Coordinate, implement, and monitor the successful transition of individuals from nursing facilities to community-based settings in accordance with Office of Aging and Adult Services (OAAS) guidelines.

  • Conduct transition assessments, evaluate individual needs and preferences, develop person-centered transition plans, and update plans as needed to support successful community living.

  • Collaborate with nursing facility staff, Medicaid Managed Care Organizations (MMCOs), support coordination agencies, behavioral health providers, and community partners to coordinate services and facilitate transitions.

  • Assist individuals with securing housing, community resources, and necessary documentation to support a safe and successful transition to the community.

  • Monitor transition progress by tracking cases, identifying and resolving barriers, facilitating transition team meetings, and providing technical assistance on complex cases.

  • Conduct pre- and post-transition visits to evaluate outcomes, address concerns, and ensure participants receive ongoing services and supports to maintain community placement.

  • Develop partnerships with community organizations, maintain resource inventories, provide education and training to stakeholders, and recommend policy improvements to strengthen transition services and community-based supports.
  •  Incumbent must adhere to pertinent laws, rules, regulations, policies, and procedures of LDH, OAAS, Civil Service and appropriate state/federal licensing and governing bodies.  Performs other duties as assigned for smooth operation of the agency.

Position-Specific Details:


PREFERRED EXPERIENCE OR TRAINING:
Knowledge of long-term care service delivery to older adults and/or people with disabilities.
Experience with one or more of the following: transition planning from nursing facilities; transition planning from ICDFs/DD; service delivery to persons with mental illness; experience with support coordination/case management; social services coordination; Medicaid or program office functions on the state or regional level; HCBS enrolled provider agency management.
