Your Specific Responsibilities:

This employee of the Florida Health Department in Miami-Dade County Epidemiology, Disease Control and Immunization Services (EDC-IS) is functioning in a supportive role under the direct supervision of an Operations Management Consultant and Assistant Community Health Nursing Director. This employee may also be called upon to function in a support role in a nurse/clerk team providing immunization services.   The incumbent will perform specialized public health work in areas geographically removed from direct supervision conducting on-site field audits of DH-680 at Miami Dade County Public Schools, Private Schools, and Daycares to verify compliance with state health standards.  Assists the Florida Health Department in Miami-Dade County Epidemiology, Disease Control and Immunization Services in achieving the Healthy People 2020 goal for immunization by assuring that at least 90% of our county’s two-year-old children successfully complete their primary series of immunizations before their second birthday.  Incumbent must be able to function independently and make decisions.

Work under the supervision of an Operations Managements Consultant, conduct field audits by reviewing DH-680 forms (Florida Certification of Immunization) in Miami-Dade County schools, private schools and daycare centers within designated zip codes to verify compliance. Schedule audit visits and maintain active communication with school and daycare administrators to track student records within FL SHOTS, while identifying out of compliance records and issue immediate notification letters to responsible parents/guardians and facility staff members.

Utilizes laptops to collect and compile data entry, and business communication. Educate facility staff on compliance as well as addresses opportunities for improvement and deliver site-level system training to educational facility personnel at daycares, private schools, and Miami Dade Public Schools to grant and manage access to the FL SHOTS network system. Notify or contact the VFC Team for any discrepancy located in the DH-680 forms, related to medical exemption for follow up and utilize company cellphones for all businesses related action during and beyond work hours, if required. Provide 30 days follow-up results for the out of compliance records. Securely upload all audit and site-visit documentation into designated TEAMS file. Generate and distribute official certificate of compliance to qualifying daycare facilities, private schools and MDCPS.  

At specific times, incumbent will function under the direction of a nurse providing immunization services in a barrier free walk-in clinic.  Duties include routine paperwork, reports, surveys and audits. Employee must be proficient in the use of computers and able to prepare and distribute reports, audits, make copies, receive incoming calls and route them to the appropriate person. Accurately transcribe and input historical immunization records from out of state and international sources in the FL SHOTS database. Perform comprehensive insurance eligibility verification for clinic patients. Create and issue valid DH-680, and DH-681 forms in strict compliance with program guidelines. Incumbent will follow Health Insurance Portability and Accountability Act (HIPPA) guidelines to protect patient privacy and data integrity.  

This employee will utilize the Florida SHOTS (State Health Online Tracking System) and Health Clinic Management System (HMS) to accurately assess, verify and input data collection from DH 687.  This electronic record should include demographical information as well as accurate documentation of the client’s immunization history and vaccinations per visit. May be called upon to function in a support role in a nurse/clerk team providing immunization services during back-to-school vaccination drives and local community outreach events.

Provides assistance to the Florida Health Department in Miami-Dade in the control of disease outbreaks by assisting in the reporting of vaccine preventable disease and the organization of mass immunization clinics.

Performs all other related duties as assigned.

Required Knowledge, Skills, and Abilities:

  • Knowledge of office procedures and practices and the ability to understand and apply applicable rules, regulations, policies and procedures.

Qualifications:

Minimum – At least one year of experience in public health, healthcare, social services, data entry, or related administrative work

Preferred – Incumbent in this position is preferred to be bilingual in English and Spanish. Experience with public health outreach, community engagement, or education
