Why does the job exist?

 
This position exists to create high performing teams at our Intermediate Care Facility to also include Supported Living homes. This position will directly oversee medical services to persons who have an intellectual and developmental disability and are residing at our Intermediate Care Facility (ICF).This position will also assist the Director of Nursing in planning, directing, coordinating and overseeing aspects of the program. Incumbent will provide direct supervision to clinical and nursing staff, as well as provide on-going training. Incumbent will educate treatment staff on medical issues, coordinate medical services, and inform physicians and treatment staff of changes in the medical condition of consumers. Additional duties may include but are not limited to, complete assessment of physical, mental and emotional needs, implementation of health care and crisis plans, complete documentation, administer medications, and treatments and follow-up. Incumbent will schedule, transports and accompany consumers to medical appointments.

How does it get done?

 
This position is completed by working hands on with the individuals served, direct care staff, and the internal and external teams.  This position will work the floor providing oversight, direction, and supervision to other Nurses and Certified Medication Aides.

Who are the customers?

 
Our customers include adults with intellectual and developmental disabilities (IDD) with a criminal history, members of their support teams, all Los Lunas Community Program (LLCP) staff, and other stakeholders. Adult individual's who are developmentally disabled and could display behavioral challenges and/or medical challenges.

Ideal Candidate

 
Experience working with developmentally disabled individuals
Experience developing and monitoring patient care plans
Experience with electronic charting
Experience working with multidisciplinary teams
Experience as a supervisor

Minimum Qualification

 
Must possess a current license to practice as a Registered Nurse in New Mexico, or eligibility for such licensure in accordance with New Mexico requirements, or eligibility for a temporary license issued for employment at a specific institution in New Mexico and five (5) years of experience as it pertains to the essential duties and responsibilities of the classification.

This position is designated as safety sensitive. Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules and regulations in relation to alcohol and/or drug testing. Employment is subject to nationwide criminal background check and is conditional pending results. Must possess and maintain a valid New Mexico Driver's License. Must possess and maintain a current Defensive Driving Certification from the State of New Mexico or must pass and receive a Defensive Driving Course Certification as a condition of continued employment. All applicants for this position must be licensed in accordance with the Nursing Practice Act S 61-3-2 through 61-3-31 NMSA 1978 and 16.12.1 through 16.12.20 NMAC, as applicable. Working Conditions

 
Work is performed in an office setting and in the homes of community based consumers with noise levels that can range from moderate to loud. Potential exposure to mentally and/or behaviorally challenged patients who may exhibit unpredictable and potentially violent behavior. Computer and phone usage. Some bending, reaching, pushing and/or pulling a maximum force 20-50 pounds and co-lifting and/or transferring individuals with an average weight of one-hundred fifty (150) pounds may be required. Potential for exposure to infectious diseases.
