This position directs and engages in data collection activities for Duval, Baker, Clay, Nassau, and St. Johns Counties for the Area 4 HIV/AIDS Program Surveillance Unit and ensures reporting of cases to the Bureau of HIV/AIDS. This position is assigned to the Florida Department of Health in Duval County and functions under the supervision of the Health Consultant B-SES (Northern Regional Consultant) in the AIDS Program Office. The incumbent is part of the global public health functions as the alternate information/key custodian for the surveillance unit. The position works in conjunction with other members of the local Disease Control team to ensure the health and safety of the community.

HIV/AIDS Surveillance:

• Establishes active liaison with medical institutions and medical specialists most likely to diagnose and treat HIV/AIDS clients in Duval, Baker, Clay, Nassau, and St. Johns Counties
• Conducts active HIV/AIDS Surveillance including reviewing medical records to determine if patients meet Center for Disease Control and Prevention (CDC) case reporting criteria.
• Assists with the development of methods to expand the system for HIV/AIDS surveillance to ensure 100% reporting of all cases
• Receives, records, tracks, and follow-ups on HIV/AIDS reports within required timeframes to ensure 90% of all HIV/AIDS cases are reported within 6 months.
• Completes HIV/AIDS reporting forms (5% or less error rate) to be sent to State Health Office (SHO) and CDC
• Develops and maintain professional relations with providers, DCHD staff, and SHO staff
• Completes field record Surveillance Tools and Reporting System (STARS) for STD follow-up as needed
• Reclassifies NIR cases when appropriate
• Assist in the completion of Surveillance data as it relates to risk exposure and provide feedback to the health officials
• Identify trends in Surveillance data to guide Prevention efforts
• Assists in the completion of Provider education materials

Establishes active liaison with local Sexually Transmitted Disease (STD) Program, Tuberculosis (TB) Program, and Counseling and Testing sites. conducts routine matches (record searches) with STARS, TB database, Health Management System (HMS), and Case Management Agencies to help identify risks and expanded risk information.

Monthly Transfer/Reports

• Ensures the timely and accurate completion of the monthly case transfer to the SHO
• Assists in the completion of reports as assigned (this includes the semi-annual report and modified local Area Slide Sets)
• Assists in the completion of data requests as assigned
• Develop and disseminate Fact Sheets for HIV Awareness Days

Security Functions as Assistant Security Manager for HIV/AIDS Surveillance unit

• Acts as alternate information/key custodian
• Assists in ensuring total security compliance within the HIV/AIDS Surveillance unit
• Assists in providing security training to staff as requested
• Assists in conducting security reviews/audits as requested

Other duties as required

• This includes, but is not limited to, the development of forms/tools, conducting in-services/training, assisting in special studies, and other related tasks
• Performs general core performance standards. Travel, special assignments and other duties and responsibilities, including those duties assigned during drills and actual emergencies/disasters. Attends related training and meetings as needed.
• Assist in special epidemiological investigations and/or studies requested by the Department of Health (DOH) and/or the Centers for Disease Control and Prevention (CDC).
• Perform other related duties as assigned.

• Takes initiative to perform inventory for laboratory supplies and orders items as needed. Performs inventory of medical supplies in exam rooms and notifies appropriate staff for ordering as soon as possible. Supplies are required to be always maintained to ensure client services are not disrupted.
• Pulls medical records for laboratory results and provider's review. Scans and attaches medical records to electronic health record.  Ensures positive communication when answering telephones and assists with telephone messages as appropriate.
• Attends in-service education, staff meetings and health promotion activities as directed or indicated, and other related duties as requested.  Ensures accurate and timely submission of Employee Leave and Attendance Records and Employee Activity Records (EAR’s). 
• Performs all other duties as assigned.

Required Knowledge, Skills, and Abilities:

This position requires that the individual possess knowledge of health care services, medical terms, epidemiology, the ability to collect and analyze data, and the ability to work independently. Additionally, the position requires the individual to possess the ability to organize and present statistical data in a logical format and the ability to identify significant statistical findings and respond appropriately. The individual must be able to utilize a computer and software including word processor, spreadsheet, publishing, and statistical applications. Other knowledge, skills, and abilities required are as follows:

• Knowledge of HIV/AIDS and STDs.
• Knowledge of methods of compiling, organizing, and analyzing data for the purpose of monitoring and evaluating. 
• Knowledge of instructional methods and training techniques including curriculum design principles, learning theory, group and individual teaching techniques, design of individual development plans, and test design principles.
• Knowledge of the structure and context of the English language including the meaning and spelling of words, rules of composition, and grammar.
• Ability to maintain confidentiality.
• Ability to effectively communicate both verbally and in writing.
• Ability to utilize problem-solving techniques.
• Ability to understand and apply applicable rules, regulations, policies, and procedures relating to surveillance practices.
• Ability to work with new material or information to grasp its complications.
• Ability to plan, organize, and coordinate work assignments.
• Ability to develop and provide in-service training programs.
• Ability to train an individual or groups of people.

Qualifications

Minimum:

• At least 2 years of experience collaborating with community-based organizations, hospitals, and/or private physician offices.
• Required to have a valid driver's license.
• Must have at least 2 years of experience collaborating with community-based organizations, hospitals, and/or private physician offices.
• Must have experience interpreting laboratory results.
• Must have experience preparing written reports.

Preferred:

• Bachelor’s degree in a health-related discipline (e.g., Public Health, Health Sciences) from an accredited college or university or a minimum of 4 years’ experience working in a communicable disease program (HIV, STD, TB).  Experience preparing written reports and interpreting laboratory results.
• Master’s in public health
