Requirements

Under general supervision, provides professional nursing care in accordance with established nurse protocols, policies, and procedures. Works in the Ryan White HIV Care clinic and program area to performs client assessments, provides treatments, educates clients on health problems and assists with the provision of the necessary referrals to internal and external health care providers. Assists with weekly and monthly vaccine inventory. Cares for population with special needs during disasters/emergencies and biological and chemical protocols. Performs other duties as assigned.

TYPICAL TASKS:
• Identifies high risk HIV patients and tailor interventions based on the needs of the population and the resources available.
• Provides treatments, administers medications/vaccines and provides preventive and maintenance care to patient/families consistent with nurse protocols.
• Screens and refers clients to other community service providers.
• Participates in the planning, organizing and evaluation of program areas to ensure the goals and objective(s) of the program are met.
• Provides clinical services to HIV population.
• Applies principles of population health to practice.
• Conducts comprehensive assessments and develops comprehensive plans of care for HIV client’s health care needs.
• Provides health care, BLS, first aid, immunizations
• Provides health education instruction and/or classes to individuals, families, for the promotion and maintenance of health.
• Develops and sustains community partnerships.
• Participates in drills and exercises on pandemic planning, mass sheltering, and mass dispensing.
• Performs job responsibilities with minimal supervision.
• May manage specific program activities and/or specific program evaluations.
• Cross training in various clinical services
• Provides additional duties as assigned.

Qualifications

Bachelor’s degree in Nursing from an accredited college or university AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification
OR 
Associate's degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR Certification.
