Your Specific Responsibilities:

This is an intermediate level professional position required to perform a full range of assignments under the general supervision of the Human Services Program Manager – SES in the Community Programs section. The primary responsibilities include mobilizing partners to address health gaps among underserved populations at a higher risk and building community capacity that includes traditional public health organizations and non-traditional partners to reach disproportionately affected populations with tailored programs and practices for healthcare strategies.

Convenes and facilitates a multi-sector community health coalition that includes members of underserved communities and organizations that serve the community. Builds cross-sectional partnerships to align public health healthcare and non-health interventions that decrease health gaps. Engages partners to collect data, report on progress, and assist with project tasks. Coordinates presentations to coalition members, updating them on the projects’ progress, issues encountered, partner recruitment, and data tracking. Coordinates/participates in activities/events that support the improvement of residents’ health status.

Supports County Health Department (CHD) activities and outreaches to increase access to health-related services to populations that are underserved. Assists with the development of all foundational plans and ensures all foundational plans include an optimal health focus. Maintains and collaborates with a CHD team consisting of staff from various program areas to administer and promote an assessment to identify professional opportunities that support the implementation of priority health area projects, as needed. Facilitates training opportunities for CHD staff and other community partners to increase their capacity to address health priority areas.

Oversees administration and reporting of county community health work plan and budget. Coordinates any contracted services related to the health improvement plan. Tracks priority area projects goals, objectives, activities and update data quarterly. Provides information as requested. Disseminates information monthly to the appropriate partners regarding training, resources, events, grant opportunities, health care access and quality, best practices, etc. Identifies grant opportunities. Maintains documentation of requested and disseminated resources.

Serves as a back-up for client satisfaction duties. Attends professional development and training.
