Under the supervision of the Specialized Health and Social Services (SHSS) Program Manager , performs complex and comprehensive public health social work to clients requesting services through the Medicaid benefit of Personal Care Services (PCS) and/or the Community First Choice (CFC) Program. Collaborating with medical providers, community partners, and other state agencies to address needs. Maintaining current and accurate documentation of all contacts/case activities in case files and designated databases according to program policies/applicable state and federal laws. Educates the public and providers about program services. Works under general supervision, with moderate latitude for the use of initiative and independent judgment.
Serves as the Children and Pregnant Women (CPW) program Liaison for Region 7, providing consultation, technical assistance, and training to prospective and current contracted CPW case management providers on the requirements and reporting expectations for the CPW program. . Routinely works with HHSC Case Management to fulfill CPW program requirements. Conducts courtesy reviews for CPW program providers and quality assurance reviews for SHSS program social workers. Coordinates quality assurance management to ensure the implementation of program policies and procedures and monitor compliance for CPW and Personal Care Services (PCS)/Commumity First Choice (CFC) programs effectiveness and quality. Assists in training new hires to SHSS program.
Conducts outreach and actively collaborates with community partners, by participating in various community organizations such as the (CRCG) Community Resource and Coordination Groups., Interagency groups as well as local coalitions. Educates the public and CPW providers, about available resources and program services in order to improve the health and well being of the community.
Other duties as assigned, which includes actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Essential Job Functions (EJFs):
Liaison to the Children and Pregnant Women (CPW) Program
Under the supervision of SHSS Program Manager and SHSS Social Work Supervisor, serves as a Program Specialist /Liaison and trainer for Case Management for Children and Pregnant Women program providers, regional case managers and potential providers. Work includes implementing recruitment, training and monitoring practices for the contracted CPW Providers and Provider agencies. Provides technical assistance to assigned social work providers as well as regional social workers. Collaborates with HHSC Case Management Division on special assignments. Conducts CPW Pre-planning Sessions with potential providers and follows them through the application process. Works in collaboration with other DSHS SHSS Liaisons from other regions as needed. Adheres to internal and external Case Management policies and procedures.
Case Manager/ Social Worker
Provides complex and specialized social work consultation and coordination of services to families of children in the Personal Care Services (PCS) Program and Community First Choice (CFC) programs. Conducts home visits with families for comprehensive person centered family interviews and assessment of home environment and family needs coordination. Connects families to other resources in the community as needed to assist family with any unmet social, emotional and physical needs. Conducts Personal Care Services (PCS) and Community First Choice (CFC) assessments in order to determine eligibility for a family with a special needs child to obtain an attendant to assist with activities of daily living. Maintains documentation of work activities with record documentation, monthly reports, travel reports, case summaries and correspondence. Utilizes software programs to do database entry, word processing, spreadsheets, electronic mail, and Internet. Collaborates with regional and central office program staff on CPW Case Management, PCS and CFC work projects. Participates in CPW case management, PCS and CFC work groups. Represents PHR 7 on teleconferences relating to Case Management Program. Assists in conducting new SHSS (Specialized Health and Social Services) staff training upon hiring of new employees. Maintains confidentiality of client information at all times. Attends staff meetings and program trainings.
Quality Assurance Specialist for Social Work cases
Work involves planning, developing and administering internal quality assurance and compliance activities for both the CPW providers as well as DSHS program social workers. Oversees QA review process and conducts comprehensive QA reviews of Personal Care Services (PCS) and Community First Choice (CFC) assessments and client database entries conducted by social services staff. Shares findings with SWII and
supervisor to ensure timely and accurate application of policies/procedures/social work best practices.
Orients SWIIs to new or changing policies/procedures and clarifies existing ones. Serves as subject matter expert and performs quality assurance in client information management systems. Identifies unmet training needs and coordinates and/or conducts supervisor-approved training sessions with SWIIs. Assists SWIIs with obtaining resources to enhance their work performance. Compiles accurate and comprehensive monthly and quarterly program reports. Communicates with SHSS Leadership staff regarding program issues, quality assurance reviews and CPW provider concerns for PHR 7.
Community Organizations and Outreach :
Conducts presentations and attends community events to promote, educate and inform the general public about CPW and other SHSS program services such as medical case management, CSHCN (Children With Special Health Care Needs) insurance program, as well as Texas Health Steps medicaid provider services. Demonstrates active participation in various community meetings such as the (CRCG) Community Resource and Coordination Groups. Communicates effectively and professionally with clients, colleagues supervisors, as well as the general public. Attends trainings and conferences relevant to social work licensure requirements and continued development of professional social work knowledge and skills.
Other Duties as assigned including Emergency Preparedness/ COOP
Assists in responding to public health needs. Maintains current required trainings . Participates as a team member in the public health response to regional and statewide disasters and emergencies, and is willing and able to carry out their role in the event of a disaster response and/or recovery effort or Continuity of Operations (COOP) activation. Will complete all required training necessary to fulfill the duties of their role, including the web-based IS-700, IS-100 and IS-200 courses accessible online at the DSHS Public Health Training (TRAIN) website within 120 days of hire date. Employee maintains an acceptable driving record that meets the standards developed by the Texas Department of Public Safety (DPS) and safely operates either a state owned vehicle or personal vehicle to conduct state business in accordance with DSHS Policy OS-3901, Fleet Operations and Management & Procedures, and Texas motor vehicle, traffic and driving licensing law.

Registrations, Licensure Requirements or Certifications:
Bachelor's degree (minimum) in social work or a related field, along with a current unrestricted license to practice in Texas as a Licensed Social Worker.
Employee must have a valid Class C Texas driver’s license, or equivalent, along with valid vehicle insurance.
Initial Screening Criteria:
Bachelor's degree (minimum) from a Council on Social Work Education accredited college or university
A minimum of 3 years working in a governmental, public health, or human services setting performing a social worker role (case management, healthcare navigation, needs assessments, and/or case reviews) is required.
Experience with Personal Care Services, Community First Choice, Children with Specialized Health Care Needs, and Texas Health Steps is preferred.
Additional Information:
DSHS is a tobacco-free workplace. This position is headquartered in Temple, Texas. An alternate headquarters within the 30-county area of Region 7 may be considered if space at an existing DSHS facility is available. If the position is headquartered outside of Temple, the person will be required to work at least one day a week in Temple. Telework is not authorized for this position. Frequent travel across the region during work hours is required.
