Community Health Worker

The Community Health Worker works to improve access to health care services; increase screening; understand between community members and the health and social service system; enhanced communication between community members and health providers. CHW provides social support and informal counseling, advocates for individuals and community health needs.

Requirements

  • Improve access to prenatal and postnatal health care services.
  • Responsible for establishing trusting relationships with participants and their families while providing general support and encouragement.
  • Effectively communicating the Perinatal and Family Center Initiative (PFCI) mission.
  • Provide ongoing follow-up, motivational interviewing, and goal setting with participants/families.
  • Follow–up with participants via phone calls, home visits, and visits to other settings where participants can be found.
  • Assist participants in understanding perinatal care plans and instructions. 
  • Provide referrals for services to community agencies as appropriate.
  • Complete required reports reflecting program services.
  • Manage assigned caseload of participants and work collaboratively and effectively within a team. 
  • Work closely with OB-GYN medical providers and Doulas to help ensure that participants have comprehensive and coordinated care plans.
  • Work collaboratively with other clinical personnel assigned to the same patient to communicate community resources appropriate to the needs of participants/families.
  • Act as a patient advocate and liaison between the patient/family and community service agencies.

Qualifications

High School diploma or GED AND Four years of progressively complex office or administrative experience.

PREFERRED QUALIFICATIONS:


Preference will be given to applicants who in addition to meeting the minimum qualifications also possess the following:

  • Have a Bachelor's degree
  • Experience collecting and analyzing data
  • Ability to multitask
  • Ability to work in a team environment
