ADVANCED PRACTICE RN SPECIALIST  - Open Competitive

Your Specific Responsibilities:

The incumbent is an Advanced Practice Registered Nurse Specialist, Autonomous APRN Licensure preferred, who works in the Clinical Health Services Division combining Quality Assurance leadership and part-time clinical practice. The incumbent will work in medical clinics, as well as various programmatic areas when needed within the scope of Agency protocols, State and Federal Guidelines. The APRN’s medical competence, medical judgment and general practice of medicine will be evaluated through Medical Director and/or peer review. Monitoring and oversight will include, but is not limited to, an established quality improvement process with ongoing chart review. The APRN’s immediate supervisor is functionally responsible for evaluation of all standards and expectations and will facilitate access to medical consultations. Specific duties are as follows. 

Administrative:

• Serves as the Clinical Quality Assurance Coordinator for the Pinellas County Health Program (PCHP) and Health Care for the Homeless (HCH) Program.
• Develops and implements QA and QI tools to monitor and measure service delivery.
• Collects, evaluates and analyzes data on performance, workflow, documentation, coding and other practice measurements.
• Provides monthly and quarterly HEDIS and UDS Clinical Quality Measure reporting for internal and external review.
• Prepares and delivers presentations and trainings.
• Collaborates with Medical Director and operational leadership to develop corrective action plans and improve workflows.
• Evaluates provider performance, including conducting audits of medical record documentation.
• Maintains the Credentialing and Privileging processes, procedures, and files.
• Ensures initial and re-credentialing for all clinical staff (licensed independent practitioners (LIPS) and other licensed or certified practitioners (OLCPs).
• Chairs the Medical Executive Committee which includes developing the agenda and distributing minutes and materials for review and approval of credentialing documents.
• Receives contractor attestations for initial and re-credentialing.
• Chairs the PCHP and HCH Quality Improvement Team and Clinical Subcommittee which includes developing the agenda and distributing minutes and materials for review and approval.
• Ensures standards and measures for National Committee for Quality Assurance (NCQA) Patient-Centered Medical Home (PCMH) recognition are met and appropriate documentation is collected and maintained.
• Acts as a preceptor for clinical trainees.

Clinical:

• Provides comprehensive primary care services to DOH-Pinellas clients in a variety of settings, including in clinic and via Telehealth.
• Evaluates and performs complete physical examinations.
• Analyzes and interprets patient histories, symptoms, physical findings, and/or diagnostic information to develop appropriate diagnoses.
• Diagnoses and treats acute health care problems such as illnesses, infections, or injuries.
• Orders, conducts, interprets, and/or evaluates diagnostic tests to identify and assess patient's condition.
• Orders, reviews and interprets laboratory tests to identify and assess patient's condition.
• Prescribes and administers medications based on patient characteristics, efficacy, safety, and cost as legally authorized.
• Makes medical diagnosis and prescribes treatment within scope of service.
• Develops treatment plans based on scientific rationale, standards of care, and professional practice guidelines.
• Makes appropriate follow ups and referrals.
• Educates patients about self-management of acute and chronic illnesses, tailoring instructions to patients' individual circumstances.
• Provides consultation and technical assistance in evaluating the appropriateness of client care as it relates to primary care.
• Complies with all selected HEDIS measures, UDS measures, standards, and selected goals for quality of care.
• Documents all services in EHR timely and accurately.
• Provides excellent customer service.
• Communicates with medical team and other colleagues.
• Completes case management, including record review and lab review, within required timeframes.
• Rotates as an on-call provider, addressing urgent medical needs of clients during non-business hours.  Follows on-call procedures and documents within required timeframes.
• Performs other related duties as required, including emergency duty. 

Required Knowledge, Skills, and Abilities:

• Knowledge of nursing principles, practices and techniques.
• Knowledge of professional nursing and public health principles.

• Knowledge of methods to measure, monitor, and evaluate performance.
• Strong organizational and analytical skills.
• Skill with computer and accompanying software programs including electronic health records and Microsoft Word, PowerPoint, SharePoint, Teams, and Excel.
• Ability to develop methods for monitoring and evaluating quality of services.
• Ability to collect, evaluate, and analyze data to monitor performance, document workflow, develop alternative recommendations, solve problems, prepare reports, and other activities relating to the improvement of operational practices.
• Ability to organize data into logical format for presentation in reports, documents, and other written materials that suit the needs of a variety of users.
• Ability to independently plan, organize, and coordinate work assignments of self and others.
• Ability to establish and maintain effective working relationships.
• Ability to communicate effectively verbally and in writing.
• Ability to assess need for and to administer medications.
• Ability to provide preventive health care and treatment to groups.
• Ability to plan and conduct a variety of training sessions.  

Qualifications:

Minimum

• Clear and active license in the state of Florida to practice as an Advanced Practice Registered Nurse.(Florida Statutes § 464.012)
• Current active Florida Nursing License or a multistate license from another compact state.
• Current CPR/BLS certification.
• Access to reliable transportation.
• Three or more years of experience working as an APRN in an Internal or Family Medicine clinic setting.
• Extensive computer skills including the use of an electronic health record (EHR).
• One or more years of experience working in quality management.

Preferred

• Clear and active license in the state of Florida to practice as an Autonomous Advanced Practice Registered Nurse.(Florida Statutes § 464.012)
• Five or more years of experience working as an APRN in an Internal or Family Medicine clinic setting.
• Three or more years of experience working in quality management.
