Social Worker Associate

The Iowa Department of Health and Human Services (HHS), Division of Aging and Disability Services , Bureau of Community Integration, Targeted Case Management is seeking a Social Work Associate  to join our team!

This position will be based in Des Moines, Iowa and will perform key duties to support a team of case managers and a supervisor who provide case management services to adults and children receiving in-home and community based services through various HCBS waivers including: Intellectual Disability (ID), Developmental Disability (DD), Brain Injury (BI), Health and Disability (HD), Physical Disability (PD) Habilitation (HAB), Acquired Immune Deficiency Syndrome (AIDS), and Elderly.   

Hours and Travel: 

Monday-Friday between 8:00am to 4:30pm. Some travel is required.


Special Requirements:


A valid driver’s license and a reliable vehicle are required for this position.


Core Job Duties: 

The duties include but are not limited to 

  • Technical and paraprofessional social work responsibilities that support the unit in providing case management services and supporting the team in a variety of administrative duties. 
  • Assists the unit in assuring that work assignments are carried out by; performing in accordance with set procedures, policies, and standards, such duties as compiling statistical or narrative information, documenting this information in report form, and preparing correspondence. 
  • Organizes and oversees files and records of a confidential, complex or otherwise sensitive nature. 
  • Evaluates work situations and exercises judgment in making technical decisions in accordance with prescribed laws and technical rules, regulations, and policies and practices. 
  • Conducts interviews to obtain necessary information for case summaries and histories, prepares fiscal reports, assessments, social histories, service plans, case records and referral summaries.  
  • Offers recommendations to professional staff for casework planning and programming.

Qualifications

Graduation from an accredited area college with an Associate's of Arts or Science Degree in Social Work, Human Services, Case Management, Public Administration, Sociology, Psychology, or Nursing;

OR
an equivalent combination of experience and education substituting one year of the required full-time experience in social work, human services, case management, public administration, sociology, psychology, or nursing for each year (30 semester hours or one year) of the required education.
