Social Worker 1-4

Division: Within the Social Services Department at ELMHS, Located in Jackson, LA

The Mission of ELMHS is to provide mental health evaluation, treatment, habilitation, and consultation for adults. Programs include inpatient interventions, jail-based evaluations, supervised group home and residential programs, forensic evaluation, care, custody, and aftercare; intermediate and long-term psychiatric services are coordinated with the total system of care in Louisiana 

ELMHS is seeking a passionate Social Worker to deliver client-centered services, including assessment, case management, crisis intervention, and community resource coordination. The ideal candidate will have experience working with diverse populations facing mental health challenges, trauma, or crises.

Requirements

Possession of a current Louisiana license to practice as a Licensed Master's Social Worker (LMSW), Licensed Clinical Social Worker (LCSW), or Certified Social Worker (CSW) plus:

A master's degree in social work.

NOTE:
Permanent status will not be granted in this job title without LMSW or LCSW licensure in Louisiana; Certified Social Worker (CSW) credential will not be accepted for granting permanent status.

Job Duties:

In this role, you will:

  • Develop an appropriate discharge and aftercare plan in cooperation with the patient 
  • Document the discharge plan on the social assessments and the treatment plan and update it as needed. 
  • Locate suitable Permanent Supportive Housing, placements in nursing, group, or licensed boarding homes for those patients unable to return home or require placement.
  • Conduct goals directed and supportive individual and group treatment with affected adult patients
  • Document Active treatment summaries on each patient chart specifying the patient's progress towards treatment objectives and goals, and response to interventions and reason for continued stay using the BIRP documentation method.
  • Collect and integrate social assessment information from patient, family, and other available resources into a complete social assessment within agency delineated timelines.
  • Participate in the development of the Master Treatment Plan using S.M.A.R.T. method
  • Participate in educational trainings that are mandatory for accreditation and licensure of the facility, including but not limited to, CPR, CPI, and First Aid. 
  • Maintain a current, valid, and unencumbered motor vehicle operator’s license.

Career Progression:  This position may be filled as Social Worker 1 – Social Worker 4 based on experience 

Work Schedule:Monday - Friday

Compensation: 

  • This position has an SER of 
    • $23.75 an hour or $1900.00 bi-weekly for Social Worker 1
    • $25.42 an hour or $2033.60 bi-weekly for Social Worker 2
    • $27.20 an hour or $2176.00 bi-weekly for Social Worker 3
    • $29.10 an hour or $2328.00 bi-weekly for Social Worker 4
  • This position is eligible for Longevity Pay starting at $2.00 Per Hour Physically Worked
