Social Service Counselor 1 - 3 (Care Coordinator)

The Social Service Counselor promotes timely access to appropriate care, increases utilization of preventative care, and reduces emergency room utilization and hospital readmissions by linking referred individuals to care, following through with referral sources, and conducting ongoing monitoring to ensure service continuity. He/she maintains in-depth knowledge of all JPHSA services and programs. He/she communicates regularly with referral sources to ensure appropriate service provision and transfer of documentation. He/ she reports to the Centralized Care Coordination Supervisor within Executive Administration and serves as part of the Care Coordination team

Requirements

Three years of social services experience; OR

Six years of full-time experience in any field; OR

A bachelor's degree.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

NECESSARY SPECIAL REQUIREMENT:
Positions that provide addictive disorder counseling or prevention services will require certification or eligibility for certification issued by the Addictive Disorder Regulatory Authority (ADRA), or its successor.

As a Social Service Counselor you will:

  • Engages individuals to gather information needed to determine appropriate service needs. Identifies and links individuals served with internal and external resources to facilitate referrals and respond to all wellness needs. Serves as the point of contact for the individuals served and their families. Ensures services are obtained through appropriate tracking and monitoring to the greatest extent possible.
  • Increases the ability of individuals served to perform self-management and shared decision-making by involvingandcommunicatingeffectivelywithindividualsserved,familymembers,caregiversandother parties involved in care both in-person and over the phone.
  • Identifies and utilizes community resources and natural supports during Care Coordination staffing, case reviews, and/or in supervision. Assists individuals served in problem-solving potential issues and barriers to their overall care and/or health (e.g. requests interpreters, transportation services or prescription assistance as appropriate).
  • Increasescontinuityofcarebymanagingrelationshipsandworkingcollaboratively with care providers, transitions-in-care, and referrals for individualsidentifiedbythereferralsourcesashighrisk,highneeds,or as directed by the Centralized Care Coordination Supervisor.
  • Assists the Centralized Care Coordination Supervisor as directed with developing and promoting adherence to interdisciplinary care plans through participation in meetings; coordinating information and care requirements with providers; resolving issues that could affect care progression; fostering support; and, providing education to others regarding the case management process.
  • Ensures accurate and concurrent documentation in Electronic Health System (EHS) by documenting communication and communication attempts with referrals as progress notes in the EHR within one (1) business day.

Appointment Type:   Probational, Promotional, or Job Appointment

Career Progression:  This position does participate in a Career Progression Group and can be filled as a Social Service Counselor 1, 2, or 3

Work Schedule: Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with some flexibility as needed to accommodate the needs of the individuals served.

Compensation: The salary offered will be determined based on qualifications and experience.  

**Note regarding the advertised pay range: The maximum amounts listed are the maximum salary amounts a person can make over their career and not the maximum amount we are allowed to pay a new hire.
