Why does the job exist?

 
The primary purpose of the Senior Social Service Assistant is to serve as a liaison between residents and the Administration and staff of the New Mexico State Veterans¿ Home.  This staff member will serve as an advocate for our residents with any concerns, needs, issues, etc., by providing a specific channel through which residents, family members and others can seek solutions to problems, concerns and unresolved needs relating to quality of life and quality of care issues.

How does it get done?

 
Essential functions of the Senior Social Service Assistant are as follows:
Conduct daily rounds of all assigned units.
Assist new admissions during orientation and adjustment to NMVH; This includes
Educate and inform residents, staff, and interested others regarding the services provided by the Senior Social Service Assistant
Notify residents of Care Plan meeting, maintain electronic version of family Care Plan calendar times and schedule afternoon Care Plan meetings with family members.
Oversee the facility Complaint Process and refer resident, staff and family complaints to the appropriate Department(s) for response/resolution, maintain a master Notebook of all complaints by the month which needs to always be Survey-ready.
Assist and support residents through the complaint/grievance process (excluding any allegations of ANE that must be investigated or personnel matters that go beyond the scope of complaints), including a response to the residents concerning findings/outcome.
Conduct a Resident and Family Satisfaction Survey a minimum of annually, and more frequently, when directed.
Maintain appropriate, confidential and timely documentation of contacts, complaints, action taken and findings.
Enter all resident requests into electronic Tracking Log as soon as possible, and a minimum of weekly.
Serve as staff liaison to the Resident Council, assisting and supporting the activities of the Resident Council, including tracking and documenting attendance at meetings and documenting minutes, posting around facility and maintaining a master Notebook that needs to always be Survey-ready.
Attend other required meetings/training courses as necessary to obtain/maintain necessary skills.
Follow up with residents in the hospital
May be asked to assist with additional tasks and projects within the facility, especially those consistent with primary purpose as identified above.
Will be responsible to back up both Resident Services Agent and the Tailor positions (when staff person is away) with time sensitive duties such as inventory, labeling, drink hosting duty, shopping needs, etc.

The primary customer are the brave men and women that answered our nations call, executed their duties and now are unable to live in their respective communities due to failing health.  This job will also support the widows of the service men and women to include parents that have lost children in combat (Gold Star Parents)

Ideal Candidate

 
The Senior Social Service Assistant must:
Have contact with every resident a minimum of once per month.
Demonstrate empathy and a willingness to assist and respond to or on behalf of residents.
Possess excellent oral and written communication skills in English.
Demonstrate flexibility in work habits and ability to prioritize effectively.
Demonstrate skill as a conflict mediator and communications facilitator.
Have effective working knowledge of personal computers, including but not limited to Microsoft Word, Excel, PowerPoint, PCC and Outlook.
Incumbent is subject to Caregiver¿s Criminal History Screening requirements.
Comply with requirements for the operation of NMVH state vehicles and be appropriately certified to provide resident transportation services.

Minimum Qualification

 
High School diploma or Equivalency and three (3) months experience as a home care worker, management of a home, or experience in providing social or rehabilitative services to clients in the community and/or in an institution. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupational totaling three (3) months may substitute for the required experience.

Employment Requirements

 
This position is designated safety sensitive.  Employment is subject to pre-employment and random testing, in accordance with all terms and conditions of federal and state law, rules, and regulations in relation to alcohol and/or drug testing.

Employment is subject to criminal background check pursuant to New Mexico Caregivers Criminal Screening Act, (Sections 29-17-2 through 29-17-5 NMSA 1978 and 7.1.9.1 through 7.1.9.11 NMAC) and is conditional pending results.

Must possess and maintain a valid New Mexico driver's license. 

Must possess and maintain a current Defensive Driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.

Successful completion of the Health Insurance Portability and Accountability Act (HIPAA) training.

Must sign and observe confidentiality agreement.

Must be able to read, write, speak, and comprehend the English language.

Occasional travel for training & meetings and driving a State of NM owned motor vehicle may be required; Working hours may include weekends, holidays, overtime, and callback hours. Essential functions of the job and specific physical ability requirements will be available at the time of interview or may be obtained at the New Mexico Veterans' Home Human Resources office.

Working Conditions

 
Work is performed in an office setting within a long-term facility with exposure to Visual/Video Display Terminal (VDT) and personal computer and phone usage. Walking throughout the facility and frequent communication and interaction with residents, staff, and the public.  Periods of walking, standing, bending pushing, pulling, lifting, and stretching for a variety of activities may be required.  Must be able to function under pressure and stressful situations Health hazards such as exposure to contagious viruses and diseases.  May also include routine exposure to public, fellow employees, and facility resident illness, stressful situation, and bodily fluids, as well as exposure to physical and/or verbal abuse. Occasional travel for training & meetings and driving a State of NM owned motor vehicle may be required; some lifting; extended hours to meet deadlines will be required. Working hours may include on-call, nights, weekends, holidays, overtime, and callback hours.  Essential functions of the job and physical ability requirements will be available at the time of interview or may be obtained at the New Mexico Veterans' Home Human Resources Office
