SENIOR COMMUNITY HEALTH NURSE

Your Specific Responsibilities:

1. Through the Communicable Disease/Tuberculosis Program this position will be responsible for the investigation, surveillance and reporting of Florida reportable disease/conditions of public health importance, TB case management/follow-up of cases, suspects, referrals and other direct nursing service in a variety of settings, e.g., home, clinic, schools, daycare centers, nursing homes, healthcare facilities, etc., utilizing the current  policies and procedures of the Department of Health in Sarasota County (DOH-Sarasota), the Florida Department of Health (DOH) and the Centers for Disease Control and Prevention (CDC). This position reports to the Operations Manager C-SES of the FDOH- Sarasota County.
   1. Conducts case management/contact investigations for Tuberculosis (TB) suspects/cases, contacts and Latent TB Infection (LTBI) clients effectively following and utilizing Florida Department of Health in Sarasota County (DOH-Sarasota), DOH and CDC policies and procedures.
   2. Participates in and/or conducts clinic services, including but not limited to:
      1. Tuberculosis nursing/MD clinics
      2. Contact screening clinics
   3. Runs TB reports monthly to monitor program activities.
   4. Assists with maintaining and updating the DOH-Sarasota TB Program Procedure Manual.
   5. Provides technical assistance to DOH-Sarasota supervisors, staff and the community regarding the TB and the TB Program.
   6. Documents physician/nursing notes in Health Management System (HMS)
   7. Maintains accurate and complete medical records.  The employer, Department of Health, County Health Department, is the owner of the medical records as defined by Section 455.667 F.S.
   8. Maintains client confidentiality in a manner consistent with Chapter 384.29, F.S. and DOH policy. 
   9. Serves as the subject matter expert for DOH Sarasota and community partners related to TB and communicable disease questions and concerns
   10. Educates client and/or family members regarding reportable diseases, disease process, and transmission and control measures
   11. Coordinates and conducts TB clinic services
2. Conducts disease investigations, surveillance, and reporting of communicable diseases. Provides or recommends disease interventions in a variety of settings, e.g., home, clinic, schools, day care centers, nursing homes, etc.
   1. Performs nursing assessments.
   2. Educates clients and/or family members regarding health, disease process and transmission.
   3. Refers and coordinates services to ensure continuity of care.
   4. Acts as contact/resource person for DOH-Sarasota.
   5. Provides educational/in-service presentations to the DOH-Sarasota staff and the community.
   6. Assists with maintaining and updating the DOH-Sarasota Reportable Disease Procedure Manual.
   7. Documents disease investigation findings in Merlin.
   8. Maintains client confidentiality in a manner consistent with Chapter 384.29, F.S. and DOH policy. 
3. Miscellaneous Activities
   1. Attends and participates in DCHP team and staff meetings, DOH-Sarasota staff meetings, and staff development activities.

Required Knowledge, Skills, and Abilities:

• Knowledge of nursing practice

Qualifications:

Minimum –

• Current, active Florida Registered Nursing license in accordance with Florida Statute 464 or a multistate license from another Compact State 
• Valid driver’s license

Preferred –

• One year or more of nursing experience
• Bachelor’s degree in nursing
• Bilingual
