Requirements
The Prevention Services Division (PSD) is actively building a workforce reflective of the diverse communities we serve. The Health Promotion and Chronic Disease Prevention (HPCDP) Branch in Prevention Services Division (PSD), reviews, plans, designs, implements and evaluates public health chronic disease interventions and related risk factors with the goal of advancing health access and improving quality of life for all Coloradans. Special emphasis is placed on evidence-based practices, data-based and evidence-informed decision making, capacity-building, partnerships, and integration between program efforts for efficiency and effectiveness. The statewide chronic disease programs within the HPCDP Branch include Heart Disease and Stroke Prevention, Comprehensive Cancer Control, Colorectal Cancer Control, Diabetes Prevention and Management, Alzheimer’s and Related Dementias (ADRD), Healthy Eating and Active Living, the Cancer, Cardiovascular and Pulmonary Disease Grants Program and the Tobacco Education, Prevention and Cessation Grants Program.
We are aware that many chronic diseases share the same causes: tobacco use, lack of access to nutritious foods, built environments to support physical activity, social connectedness among others. Our work will continue until all Coloradans have the opportunity to live and have healthy lives and we work to achieve this by decreasing the burden of disease, supporting social needs, and growing health care access across Colorado through all of our programs and practices.
This position is housed in the Diabetes, Cardiovascular Health and Alzheimer’s Disease and Related Dementias Unit (CarDiADRD Unit) which focuses on engaging with health systems, community-based organizations and other partners to improve chronic disease outcomes for Coloradans. The unit also collaborates with internal programs including those focused on cancer, healthy eating and active living, tobacco, informatics, among others, to support a comprehensive system of partnerships that address specific chronic disease needs in innovative and sustainable ways.
What You Will Be Doing:
This position serves as the subject matter expert on advancing measurable improvements in population health outcomes through a Quality Improvement (QI) approach. The position provides strategic and technical expertise to help clinics, community-based organizations, and state partners strengthen implementation of evidence-based interventions using proven QI methods, independently adapting and applying established theories, standards, and models to fit varied organizational and community contexts. This position possesses knowledge to work across the continuum of quality improvement initiatives related to cardiovascular health (defined as heart health, diabetes prevention and management and brain health) including: advancement of health information technology (HIT) ande-referral infrastructure, patient access to prevention and management and social support services delivered in community or clinical settings, and health system implementation through a quality improvement approach.
Specifically, this position collaborates with internal programs and external entities possessing an interest in advancing cardiovascular health through quality improvement strategies in Colorado. Internal programs include, but are not limited to, diabetes, cardiovascular health and Alzheimer’s Disease and Related Dementias, cancer, tobacco, healthy eating and active living, and the Center for Health and Environmental Data. External entities include, but are not limited to, Department of Health Care Policy and Finance (HCPF), Colorado Rural Health Center, Colorado Department of Labor & Employment, Colorado’s Regional Accountable Entities, Trailhead Institute, Colorado Health Institute, Colorado Community Health Network, Carina Health Network, The University of Colorado School of Medicine, health systems, community-based organizations, and local public health agencies.
The position is responsible for collecting and analyzing data, examining lessons learned from other states and identifying opportunities that enable CDPHE to achieve long-term goals between public health, payers, health systems and technology partners. It requires the analysis of complex and sometimes incomplete information to determine appropriate clinical quality improvement processes. This position identifies areas in which internal programmatic efforts and external entity services or operations can be aligned with CDPHE’s strategic clinical? quality improvement and population health efforts. It identifies gaps in existing policies that have the potential to create access gaps to cardiovascular health and social support services and proposes solutions to these challenges. The position communicates CDPHE’s programmatic goals to external entities and communicates engagement opportunities to internal programs. This position provides input on related funding applications and awards, and environmental scans; facilitates stakeholder engagement, synthesizes and shares existing resources; and advises on clinical protocols and procedures.

MINIMUM QUALIFICATIONS:
An applicant may meet the minimum qualifications for this job in one of two ways: They may qualify through their work experience alone or through a combination of education and work experience.
Experience Only:
Six (6) years of professional experience in public health, health systems improvement, healthcare quality improvement, evidence-based intervention implementation, evidence-based disease management and/or prevention in public health or in a clinic or other health-related organization. This includes supporting team-based care models, clinical workflow development, and using data to inform improvement strategies.
OR
Education and Experience:
A combination of related education, graduation from an accredited college or university in public health, health promotion, health policy, healthcare administration, epidemiology, health sciences, social work, nursing or a field of study closely related to the work assignment and professional experience in public health, health systems improvement, healthcare quality improvement, evidence-based intervention implementation, evidence-based disease management and/or prevention in public health or in a clinic or other health-related organization. This includes supporting team-based care models, clinical workflow development, and using data to inform improvement strategies equal to Six (6) years.

In addition to the minimum requirements, the successful candidate in this position will have the following experience:
- Experience implementing or supporting healthcare quality improvement initiatives, particularly those focused on cardiovascular disease, diabetes, or other chronic disease prevention and management.
- Experience with team-based healthcare models, specifically those facilitating collaboration among physicians, pharmacists, nurses, community health workers, and/or community-based providers.
- Experience working with or supporting health systems, clinics, provider networks, accountable care organizations, or community-based health programs.
- Experience using data, quality metrics, or electronic health record (EHR) data to identify care gaps and inform improvement strategies.
- Experience facilitating or participating in learning collaboratives, stakeholder meetings, or cross-sector partnerships involving healthcare providers, community organizations, or state agencies.
- Experience supporting community-clinical linkages, referral systems, or partnerships between healthcare providers and community organizations.
- Demonstrated experience managing complex projects, including establishing timelines, coordinating partners, monitoring progress, and reporting outcomes.
- Demonstrated ability to translate complex technical or public health concepts into practical implementation strategies for diverse partners.
- Strong relationship-building and collaboration skills with diverse partners including healthcare providers, community organizations, and government agencies.
- Experience applying public health principles to improve access to preventive care and supportive services.
CONDITIONS OF EMPLOYMENT
- Pursuant to CDPHE policy, a background check is required for all positions at CDPHE.
TRAVEL: 10% of time; travel is predominately in-state to meet with local stakeholders, partners and grantees, with occasional out-of-state travel to meet with national partners. Local travel is approximately 2-5 times monthly; out of state travel is approximately two times per year. Travel is predominately in the metro Denver area with some occasional travel to rural Colorado; out of state travel is most often to conferences or national meetings and the location varies.
