This is a highly sensitive, independent and responsible position that supervises the Epidemiology program at FDOH-SJC. This position is responsible for surveillance and disease control activities for the reportable diseases and conditions in St. Johns County.
The Epidemiology Program is responsible for surveillance and disease control activities of over 100 reportable diseases and conditions in St. Johns County.
This position spends the majority of the time engaged in supervisory responsibilities. This includes communicating with, motivating, training, and evaluating employees in addition to planning and directing employees' work. The incumbent has the authority to hire, transfer, suspend, lay off, recall, promote, discharge, assign, reward, or discipline subordinate employees or effectively recommend such action. The incumbent will also be required to develop performance expectations, meet with staff regularly to discuss performance, complete performance reviews, maintain vacation and work schedules, ensuring proper coverage, and complete timesheets.
This position serves as a lead in active outbreak investigations and disease control activities by:
- Serving as the local subject matter expert
- Coordinating necessary tasks and staff to complete those tasks
- Coordinating efforts with other FDOH-SJC programs
- Managing emergency preparedness plans in coordination with the FDOH-SJC Planner
- Participating in necessary trainings to remain current on epidemiological related information
This position is responsible to ensure the completion of and for reportable illness case investigation duties that include but are not limited to:
- Conducting case investigations of assigned reportable illnesses and outbreaks following the Surveillance and Investigation Guidance.
- Interviewing individuals by telephone and, when necessary, in the field.
- Providing direct client information and education.
- Collecting information on appropriate case contacts for the purposes of testing, immunizing, and/or educating.
- Collecting specimens when appropriate and necessary.
- Completing Disease Intervention Specialist training.
This position is responsible to ensure completion of and complete epidemiology reports. This process includes but is not limited to:
- Contacting physician offices, laboratories, hospitals, and clients for information.
- Conducting medical record reviews as needed.
- Providing basic reporting information to medical providers when appropriate.
- Communicating with local agencies, as appropriate, in relation to disease spread and investigation.
- Coordinating with Environmental Public Health; health care agencies/providers; local, county and state partners; and additional disease control programs.
- Conducting regular surveillance for disease trends.
- Completing necessary forms, entering data into appropriate systems, and ensuring proper documentation.
This position is responsible for the completion of and completing epidemiology case investigations of Sexually Transmitted Diseases (STD). This process includes but is not limited to:
- Interviewing individuals for investigating purposes as well as notifying, counseling, referring, and educating individuals related to case investigations.
- Coordinating with appropriate departments and agencies as necessary.
- Providing disease intervention and public health awareness related to STDs to internal and external partners as appropriate.
- Completing HIV 500/501 certification training.
This position is responsible for timely and accurate data input and analysis within the Epidemiology Program which includes but is not limited to:
- Entering accurate information into the necessary and appropriate systems and updating as needed.
- Inputting protected health information as needed into necessary and appropriate systems.
- Monitoring appropriate systems for community health trends and disease and syndrome clusters.
- Analyzing surveillance information and reporting appropriately.
- Completing necessary reports in a timely manner.
This position is responsible to ensure the completion of and for rabies case and rabies possible exposure case investigation duties that include but are not limited to:
- Conducting case investigations of assigned rabies cases according to the Surveillance and Investigation Guidance including the Rabies Guidance Documents.
- Contacting physician's offices, laboratories, hospitals, Veterinarian offices and clients for information to complete case investigation.
- Performing record reviews on hospital records when necessary.
- Providing basic information on reporting to physician offices or clinics when appropriate
- Interviewing individuals by telephone and, when necessary, in the field.
- Providing direct client information and education which includes prevention.
- Compiling data to develop reports on rabies investigations.
This position is responsible for the creation, updates and reviews of all policies and procedures and quality assurance/quality improvement activities for the Epidemiology Program.
This position may be deployed and/or asked to assist other counties and agencies at the request of FDOH-SJC in support of epidemiological investigations, surveillance activities, public health investigations, public health emergencies and/or public health disaster response and recovery efforts.
Performs other duties as assigned.

Other job-related requirements for this position: Incumbent may be required to work before, during and/or beyond normal work hours or days in the event of an emergency. Emergency duty required of the incumbent includes working in special needs or Red Cross shelters, or performing other emergency duties including, but not limited to, responses to or threats involving any disaster or threat of disaster, manmade or natural.
