ABOUT NACDD
The National Association of Chronic Disease Directors (NACDD) is a member-based Association that improves the health of the public by strengthening state-based leadership and expertise for chronic disease prevention and management. NACDD’s core membership is composed of the 59 State and Territorial Health Department Chronic Disease Directors and their staff who protect the health of the public through primary and secondary prevention efforts and work upstream on root causes of chronic conditions. In addition, NACDD unites 7,000 chronic disease professionals across the United States working in state, tribal, and territorial health departments; nonprofits; academia; and the private industry to promote health and reduce the burden of chronic disease. Position Overview
NACDD is hiring three (3) Program Managers, Diabetes to support CDC-funded initiatives within our Diabetes Portfolio. We are filling multiple Program Manager positions under a single posting, each focused on a distinct strategic area of diabetes prevention and care.
All Program Managers will:
- Lead multi-partner, grant-funded projects
- Coordinate subject matter experts and vendors
- Manage workplans, budgets, and CDC reporting
- Facilitate learning communities, technical assistance, and convenings
- Collaborate closely with evaluation and communications teams
Program Focus Areas
1) Health for All (Equity- and Community-Focused Programming)
This role centers on health equity, tribal partnerships, and culturally responsive approaches to diabetes prevention and management.
Key project areas include:
- Prediabetes health marketing and outreach
- Diabetes prevention support for tribes
- Technical assistance for Good Health and Wellness in Indian Country (GHWIC) recipients
Best fit for candidates with experience in:
- Working with tribes, tribal organizations, or Indigenous-serving entities
- Community-driven health promotion and health communications
- Equity-focused public health initiatives
2) Innovating Coverage & Care (Employers, Systems, and Community Models)

3) Public Payment Solutions (Medicare & Medicaid)
This role focuses on public payer coverage and reimbursement strategies for diabetes prevention and self-management.

- National DPP Medicare coverage capacity building
- Medicaid coverage and enrollment strategies
- DSMES coverage under Medicare and Medicaid

- Technical assistance related to public payer implementation
Core Responsibilities (All Roles)
- Manage multiple CDC-funded projects from planning through reporting
- Develop and track workplans, timelines, and budgets
- Coordinate contractors, subject matter experts, and partners
- Lead meetings, trainings, and peer-learning activities
- Produce high-quality deliverables and required reports
- Collaborate across NACDD teams to align strategy and impact
- Identify opportunities for cross-project learning and synergy
Minimum Qualifications
- Bachelor’s degree in public health, health sciences, or a related field
- 5–7 years of experience managing chronic disease prevention or health promotion programs
- Experience working on CDC- or federally funded initiatives
- Demonstrated project management and facilitation expertise
- Strong written and verbal communication skills
- Ability to manage multiple projects concurrently in a remote environment
Preferred Qualifications (Varies by Focus Area)
- Master’s degree in Public Health or related field
- Experience in:
- Tribal or Indigenous health (Health for All)
- Employer-based or systems-level initiatives (Innovating Coverage & Care)
- Medicare/Medicaid coverage and payment (Public Payment Solutions)
- Prior experience in state or local health departments or member-based organizations
