CLHSD provides mental health, addictive disorder, and developmental disability services across Avoyelles, Catahoula, Concordia, Grant, LaSalle, Rapides, Vernon, and Winn parishes. We are seeking a Practical Nurse—Licensed 1 or 2 to be the first point of medical contact, creating a welcoming environment while completing health screenings and medical histories. Requirements

Possession of a current Louisiana Practical Nurse license, a temporary permit, or a multi-state license issued by a Nursing Licensure Compact (NLC/eNLC) state.

Clinical Nursing Duties:

  • Conduct thorough medical histories for clients receiving Addictive Disorder Services.
  • Perform routine nursing procedures, including administering injections and assisting with client preparations.
  • Monitor and document vital signs, including weight, BMI, blood pressure, pulse, temperature, allergies, family history, and current medications.
  • Ensure laboratory results are available and complete all required clinical documentation in accordance with clinic standards.
  • Collaborate with the RN on significant or questionable client findings.
  • Request and organize medical records for prescribers, and facilitate communication with community providers to support coordinated care.
  • Provide crisis assessments in the absence of another licensed professional.
  • Prepare client discharge summaries to support continuity of care.

Position-Specific Details:

Appointment Type: Probation.

Location: Behavioral Health Clinic/Caring Choices Clinic-Alexandria.  

Work Schedule: Monday - Friday, 8 am - 4:30 pm.

Career Progression: This position may be filled as an LPN 1 or 2.

Compensation: Eligible for premium pay of one dollar ($1.00) per hour premium for hours worked.

**Note regarding the advertised pay range: The advertised maximum amount listed is the maximum salary a person can make while in this title/level over their career and not the maximum amount we are allowed to pay a new hire.
