BRIEF JOB DESCRIPTION:
Performs complex (journey level) public health nursing work for Public Health Region 2/3 under general supervision of the Assistant Regional Nurse Manager, Regional Nurse Manager and Regional Medical Director with moderate supervision and limited latitude for the use of initiative and independent judgement. Responsible for case management of perinatal hepatitis B cases in Public Health Region 2/3. Works directly with the Regional Perinatal Hepatitis B Coordinator, local OB/GYN and pediatric providers and families to provide delivery of services and nursing consultation. Provides direct public health nursing clinical services under Standing Delegation Orders (SDO’s) for immunizations, tuberculosis and HIV/STI Adult Health. Utilizes the nursing process in assessing, planning, implementing, and evaluating public health services. Serves as a resource on nursing practice and education. This position will develop and provide training for public health nurses in compliance with nursing standards, state laws, federal/state regulations and current SDO’s to ensure a competent workforce. Will provide written reports/evaluations of training for individuals and groups and makes recommendations for corrections of deficiencies or appropriate progression. Member of the Incident Command Structure in response to public health emergencies. This position is office-based but must be able to independently travel throughout the State of Texas to complete essential job functions. Must comply with HHS, DSHS, Texas Department of Public Safety applicable policies/procedures/standards. This position is office-based, Monday-Friday, 8:00 am – 5:00 pm.
ESSENTIAL JOB FUNCTIONS: Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
CASE MANAGEMENT (40%):
- Performs case management activities related to perinatal hepatitis B prevention (PHBP) in Public Health Region 2/3.
- Works closely with Regional Perinatal Hepatitis B Coordinator and physicians to identify HBsAg positive pregnant women by reviewing lab reports to determine program eligibility. Educates families on hepatitis B transmission, case management process and follow up for contacts. Assures postpartum serology testing is completed on mothers.
- Performs case management of infants born to HBsAg positive and status unknown mothers including susceptible household and sexual contacts. Assures infants born to these mothers receive the hepatitis B immune globulin (HBIG) and the first hepatitis B dose within 12 hours of birth. Assures infants receive the hepatitis B vaccine according to schedule. Assures serology testing of infants and contacts to determine infection status.
- Documents all case management activities in the appropriate reporting and case management database for submission to DSHS Central Office.
- In collaboration with the Regional Coordinator, may aid in assessing community needs. May provide approved educational activities using state approved training presentations and brochures for health professionals to include prenatal care providers, hospitals, birthing centers, pediatric providers and clinics, and laboratories on hepatitis B transmission, reporting laws, vaccination schedule, case management, surveillance methods and contact investigations.
TRAINING AND COORDINATION (30%):
- Responsible for the orientation and training of public health nurses.
- Plan the orientation calendar in conjunction with the Nurse Supervisor and submit it to the Assistant Regional Nurse Manager (ARNM) for approval.
- Provide training in compliance with nursing standards, state laws, federal/state regulations, and current SDO’s using regionally approved orientation standards and checklists.
- Provide interval updates of training progress and make recommendations for correction of deficiencies to the ARNM.
- In collaboration with PHN leadership, may participate in training or coordination of training for regional public health staff in Bloodborne Pathogen Exposure, handling and shipping of biological specimens, fit-testing for N-95 respirators, CPR and other occupational training as identified.
- Provides consultation, training and technical assistance to agency staff and contracted people providing directly observed therapy for tuberculosis clients.
DIRECT SERVICE PROVISION (20%):
- Provides public health nursing services in the clinic, home, and community.
- Utilize components of the nursing process in assessing, planning, implementing, and evaluating the physical, emotional, and psychosocial health status of clients, families, and communities.
- Provide individualized care according to policies, procedures, program standards, physician orders and SDO’s which may include lab testing, home visits, providing/administering medications, education and counseling, case management, referrals, and follow-up.
- Performs communicable disease intervention, surveillance, and prevention functions around Tuberculosis, HIV/STD, Immunizations, and other identified diseases.
- Documents service delivery using available electronic platform and agency approved program forms.
COMMUNITY PREPAREDNESS/OTHER DUTIES (10%):
- Applies epidemiological principles and methods to assess, prevent, and control disease and promote health.
- Actively participates and/or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery as well as any activation of the Continuity of Operations Plan which could require an alternate shift assignment and/or work location.
- Participates in other regional assignments or program related duties and special initiatives, including activities requested by Central Office.
- Maintains compliance with all DSHS Policies including but not limited to Fleet Operations Management & Procedures and Standards including holding a current valid Texas Driver's License.
- Maintains a valid and current Registered Nursing License from the Texas Board of Nursing or recognized compact state.
KNOWLEDGE, SKILLS AND ABILITIES (KSAs):
- Knowledge of and ability to comply with Texas Department of State Health Services (DSHS) confidentiality guidelines.
- Knowledge of nursing techniques and procedures; public health laws and regulations; community health and nursing care principles, practices, and procedures; and quality assurance principles and procedures.
- Knowledge of hepatitis B virus, perinatal transmission including screening guidelines, case management procedures and contact tracing.
- Knowledge of various teaching methods, learning styles and types of leadership.
- Skill in using reasoning to identify problems; considering appropriate resolution of situations; collaborating with patients, families and the community to address community needs; providing program specific and public health information.
- Skill in case analysis and providing clinical case management services; communicating effectively with others (internal / external) both orally and in writing; delivering constructive feedback.
- Skill in performing phlebotomy; obtaining and processing specimens; administering medications and immunizations; using a computer and applicable software programs.
- Ability to interpret public health laws and regulations; utilize components of the nursing process to evaluate the physical, emotional, and psychosocial health status of clients, families, and communities; plan, coordinate, and evaluate the delivery of direct public health services.
- Ability to conduct contact investigations; provide an encouraging learning atmosphere; provide patient care; prepare and maintain records.
- Ability to apply time management principles to tasks; maintain effective working relationships using tact and diplomacy; work independently exercising sound judgment; work well and effectively in a multidisciplinary team environment; understand and follow written and oral instructions.

REGISTRATIONS, LICENSURE REQUIREMENTS OR CERTIFICATIONS:
- Must possess a valid Class C Texas driver's license, or equivalent license from another state with the intent to obtain a Texas driver's license.
- Must be currently licensed to practice as a Registered Nurse in the State of Texas or a state that recognizes reciprocity through the Enhanced Nurse Licensure Compact.
INITIAL SCREENING CRITERIA:
- Graduation from an accredited school of nursing.
- A minimum of one-year experience in nursing, 3 years, preferred.
