Requirements

Job Description: Under general supervision, provide direct, client centered nursing care to clients exclusively enrolled in the Ryan White program which includes: screening patients for eligibility, medical case management including treatment adherence, assessing for barriers to care, evaluation, counseling,  maintaining accurate documentation, and providing a broad range of services that link clients to sub-specialty services.  Must feel comfortable providing care for a culturally diverse population with varying lifestyle choices and provide sexual health education and promotion.  

Qualifications

Minimum

Qualifications: Associate degree in nursing from an accredited college or university and one (1) year of nursing experience and completion of a PHN course within one year of hire and current Georgia Registered Professional Nurse license and CPR certification; or two (2) years of experience at the lower-level PH Registered Nurse 2 (HCP191) or position equivalent.   Bachelor’s degree preferred.
