As a PH Registered Nurse within the North Georgia Health District at the Georgia Department of Public Healthyou will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.


Requirements

  • Assures Children's Medical Services program operations include the accurate processing of paperwork, documentation and record keeping of protected health information
  • Provides care coordination and case management services to children who have complicated medical conditions
  • Assesses the patient's condition, develop plans of care, educates patient and the family and refers patient to agencies that can assist in meeting health and/or social needs
  • Documents pertinent observations and responses to treatment
  • Conducts home visits to complete comprehensive assessments of client needs
  • Assists with monthly Orthopedic Clinics and quarterly Genetics and Neurology Telehealth Clinics
  • Facilitates physician/patient communication and treatment via Telehealth
  • Assists 6 counties in District 1-2 in the promotion and enrollment of eligible children into the Children's Medical Services and Children First program
  • Develops and sustains community partnerships
  • Participates in program outreach
  • Conduct presentations in schools, churches, organizations and etc.
  • Works closely with other programs within CWSN Unit such as Children First, Early Hearing Detection & Intervention, and Babies Can't Wait to ensure linkage to all eligible services
  • Screen and refer clients to other community service providers
  • Participate in drills and exercises on mass dispensing
  • Assist in DPH current response to pandemic
  • Performs job responsibilities with minimal supervision

Qualifications

Associate's degree in nursing from an accredited college or university AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. Note: No experience required.

Preferred Qualifications:
Bilingual- English/Spanish
Experience in providing case management services
Public Health experience
Baccalaureate level health assessment course

Current State employees are subject to State Personnel Board rules regarding salary.


CBH accepts educational credential recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) will be considered.


All positions require criminal history record check. Required education credentials, license, certification and/or registration must be submitted prior to employment in the position. Some positions require pre-employment drug testing in which the applicant who is offered the position will be required to successfully complete.
