Children with Special Health Care Needs (CSHCN) Registered Nurse position will provide medical Care Coordination and nursing care to clients with chronic and disabling complex medical conditions, Foster children with inherent trauma exposure, direct services in clinic settings, home setting, face to face visits, daily extensive contact with medical providers, specialists, physicians, pediatricians, collaboration with WVU and Marshall medical services, RN will provide Care Coordination to CSHCN enrolled in the CSHCN program for nutrition services, act as a bridge for the client/families linking medical homes and services, pediatric specialist and those with critical needs, RN will lead the CSHCN Team in completion of the medical component of the Plan of Care with medical homes, MCO's and DH programs, external partners sharing in the medical care of the CSHCN client, medical vendors for equipment and supplies, but mainly advocating for the complex child to ensure all aspects of the health care needs are met and that a full comprehensive medical and holistic assessment is available and completed for the client.

This RN position is critical for the continuation of nursing and health services to the enrolled CSHCN for WV at-risk pediatric populations that are served. The RN will provide Registered Nursing services through the Team-based health care delivery model to advance integrated care systems for CSHCN with a national mandate under Public Law 101.238. As an RN in the CSHCN program, this position serves in a gap filling capacity as a medical resource in rural areas of WV that have provider shortages and decreased ability for access to health care or able to obtain care.

Qualifications

Training: Baccalaureate degree in nursing from an accredited four-year college or university and two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program and three years of experience as a registered professional nurse.
OR
Professional certification from a recognized national nursing certifying organization as a nurse or nurse practitioner in the area of assignment plus two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program; plus baccalaureate degree in the health sciences or behavioral sciences and two years of full-time or equivalent part-time paid experience in nursing.
OR
Master's degree in nursing from an accredited four-year college or university.
 
Special Requirement: Current West Virginia license or temporary permit to practice as a registered professional nurse.
