The mission of the Louisiana Department of Health is to protect and promote health and to ensure access to medical, preventive, and rehabilitative services for all residents of the State of Louisiana.



Be part of the team that ensures critical services reach individuals across the state. The Program Operations Division oversees statewide Home and Community-Based Services (HCBS), Nursing Facility Admissions, Support Coordination, Provider Relations, and Training through nine regional offices.


This role, within Regional Waiver Operations, plays a key part in managing and supporting field activities for eligible HCBS participants. Responsibilities include program administration, monitoring, eligibility oversight, training, and technical assistance to ensure high-quality, compliant service delivery.

Requirements

A current Louisiana license in a health services field, health regulation field, or social services field plus two years of experience in hospital or nursing home administration, health services, health regulation, or social services; OR

A bachelor's degree plus two years of experience in hospital or nursing home administration, health services, health regulation, or social services; OR

A master's degree in a health services field, health regulation field, or a social services field plus one year of experience in hospital or nursing home administration, health services, health regulation, or social services.

Job Duties:
  • Administers and oversees the Community Choices Waiver (CCW) and Adult Day Health Care (ADHC) Waiver programs to ensure services are delivered in accordance with federal 1915(c) waiver requirements and OAAS policies.
  • Ensures participants receive appropriate services based on standardized assessments, supporting individualized plans of care that promote community-based living as an alternative to nursing facility placement.
  • Monitors waiver operations to ensure program efficiency, regulatory compliance, fiscal accountability, and quality service delivery for seniors and individuals with adult-onset disabilities.
  • Serves as the Local Contact Agency (LCA) for the CMS-required MDS 3.0 Section Q process by responding to nursing facility referrals, conducting Nursing Facility Level of Care (NFLOC) determinations, and facilitating waiver offers and community resource referrals when appropriate.
  • Completes Nursing Facility Level of Care determinations for admissions, continued stay requests, temporary nursing facility stays, and participants involved in the My Choice Louisiana program.
  • Conducts annual monitoring reviews of OAAS Support Coordination Agencies (SCAs) to evaluate service quality, compliance with program requirements, and effectiveness of participant-centered service delivery.
  • Provides technical assistance and guidance to Direct Service Providers (DSPs) and Support Coordination Agencies (SCAs) to ensure adherence to program rules, strengthen service delivery, and promote positive participant outcomes.
