The New Mexico Behavioral Health institute - Meadows Long term Facility is located on Las Vegas, New Mexico. It provides long-term and skilled nursing service. the facility has 162 nursing home, skilled-nursing beds. Services for residents are provided 24/7, focusing on maintaining or improving their physical, mental, and psychosocial well-being under a personalized plan of care developed by licensed health professionals.

Interviews are anticipated to be conducted within two weeks of closing date.

Why does the job exist?

 
The purpose of this position is responsible for adequately assessing nursing home residents' needs, and coordinating personalized, resident-driven care based on those assessments. The MDS contains items that measure physical, psychological and psycho-social functioning. The MDS plays a key role in the Medicare and Medicaid reimbursement system and in monitoring the quality of care provided to nursing facility residents.

How does it get done?

 
Assures each new resident has a current RAI completed within required time frames
a) Assesses each new resident using the RAI process completed within the required time frames
b) Prepares data for MDS entry to ensure MDS and CAAs are complete prior to initial treatment team meeting, quarterly and annually
c) Participates with treatment teams to complete an interdisciplinary care plan for each resident ensuring all required time frames are met.

The primary customer current and future residents who are now unable to live in their respective communities due to failing health.

Ideal Candidate

 
The Ideal Candidate would have:

-The ability to read, write, speak, and understand the English language.

-The ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.

-Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities.

-The ability to function independently, and have flexibility, personal integrity, and the ability to work effectively with residents, personnel, and support agencies.

Minimum Qualification

 
Must possess a current license to practice as a Registered Nurse in New Mexico, or eligibility for such licensure in accordance with New Mexico requirements, or eligibility for a temporary license issued for employment at a specific institution in New Mexico and five (5) years of experience as it pertains to the essential duties and responsibilities of the classification.

Must possess a valid driver's license.

Must obtain a Driver's Defensive Certificate.

Employment is conditional pending the results of criminal records background checks. Must possess and maintain a valid driver's license. Successful completion of the Health Insurance Portability & Accountability Act (HIPAA).

Working Conditions

 
Work is performed in an office setting with exposure to Visual/Video Display Terminal (VDT) and extensive personal computer and phone usage.
