Maintenance Worker/Mobile Health Care Clinic Driver
OUTCOME 1: MAINTENANCE WORKER
CONTRACTOR shall serve as a Maintenance Worker for Northeast Delta Human Services Authority(NEDHSA).
- Daily maintenance of buildings, grounds, and vehicles; making necessary repairs and reporting to immediate supervisor and CFO as soon as problems have been identified. Ensure routing maintenance such as; painting, minor construction projects, repair/replacement of tiles, door locks, window screens, furniture, doors, equipment, etc; inspect roofs and remove debris and leaves from roofs and drains.
- Ensure exterior of building and grounds are up to date and presentable. Keep limbs trimmed around electrical wires. Keep openings to drainage ditches cleaned. Cut grass, trim shrubs and all other duties requested within your scope of work.
- Assists safety officer at R.I.S.E. Treatment Center. Attends Regional Safety meeting and participates in ORM training and other trainings as requested. Inspects building for safety hazards, making sure all safety requirements are met and co-workers are informed regularly of safety issues. Conduct inspections on a timely basis and complete paperwork as required. Ensure all work orders are maintained and updated via ASANA.
- Inspects state vehicles, maintain fluid levels, keeps serviced and ready for use, make minor repairs, and insure preventive maintenance is completed according to state vehicles guidelines.
OUTCOME 2: DRIVER
The CONTRACTOR shall serve as R.I.S.E. Treatment Center and Mobile Health Care Clinic Driver for Northeast Delta Human Services Authority(NEDHSA).The CONTRACTOR shall act as a Mobile Health Care Clinic Driver as someone who is responsible for driving the Mobile Health Care Clinic to specified locations, as needed. The driver should utilize a navigation system to assist in mapping out the destination. Drivers may have to work nights and weekends to accomplish their duties.
The R.I.S.E. Treatment Center and Mobile Health Care Clinic Driver is to be licensed to operate this vehicle type in the State of Louisiana. The incumbent will complete NEDHSA employee orientation/training as scheduled.
CONTRACTOR shall be responsible for maintenance and service of Mobile Health Care Clinic, including routine maintenance and repairs. All must be coordinated with NEDHSA maintenance and fiscal department staff. All services performed must be documented and reported to NEDHSA Contract Monitor. CONTRACTOR will supply NEDHSA with a final invoice and a completed report within 2 days after the vehicle assessment is completed. CONTRACTOR will not perform any Maintenance Services until it receives written authorization from NEDHSA, and then only in accordance with the terms and provisions of the agreement between the two parties. Compensation for any repairs will be invoiced to NEDHSA, provided approval was granted.
Record keepingCONTRACTOR shall maintain records for service routes and all mileage and safety records.
CONTRACTOR shall ensure that all rules of confidentiality pertaining to HIPPA and/or medical records are observed during Mobile Health Care Clinic operation.
