Why does the job exist?

 
Provide case work/ social work services to residents of the assigned unit. This included being the primary contact person for family members and or guardians to our long term care (LTC) Unit. Unit is ASPEN.

How does it get done?

 
Incumbent will provide social work services for NMBHI/LTC and is responsible for clinical assessments and discharge planning as needed and provide clinical services and medical social services to residents who are being admitted to the "Long Term Care Division" as well as their guardians and family members.

Residents in the Long Term Care Division.

Ideal Candidate

 
Ideal Candidate will have experience working with treatment planning; experience working with providing direct effective treatment to patients; experience working with various psychotherapeutic modalities, and experience working with mental illness.

Minimum Qualification

 
Associate's Degree in Human Services or Social Services Counseling and two (2) years of experience in mental health, substance abuse counseling and or behavioral disorder counseling environment. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.

Must possess a current driver's license and defensive driving certificate.

2.CRIMINAL RECORDS BACKGROUND CHECK-Must successfully complete post offer pre-employment criminal records background check. Employment is conditional pending the results of criminal records background checks. Must possess and maintain a valid driver's license. Successful completion of the Health Insurance Portability & Accountability Act (HIPAA).

Working Conditions

 
This position will perform work in a hospital and office setting with extensive computer and phone usage. Exposure to physically, mentally and behaviorally challenged residents who may exhibit unpredictable and sometimes aggressive behaviors in volatile situations required.
