Your Key Responsibilities:
Patient Care:
- Take and give comprehensive reports during shift changes to ensure seamless patient care and continuity.
- Count and document medications accurately, ensuring compliance with safety protocols.
- Assist residents with self-medication programs, providing guidance and support to promote independence and health management.
- Basic wound care.
Documentation:
- Accurately document any changes in residents’ status or behavior, providing timely updates to the medical team.
- Regularly monitor and record vital signs to ensure residents' health and well-being.
- Perform and document fingerstick blood sugars (FBS), interpreting results and reporting any concerns
Emergency Preparedness:
- Assist in evacuations during fire alarms, ensuring the safety and well-being of all residents.
- Follow established emergency protocols and participate in regular drills to maintain readiness.
Collaboration:
- Maintain effective communication with VAMC physicians, nurses, and pharmacy staff regarding residents’ well-being and medication management, and appointments.
- Participate with interdisciplinary teams to discuss resident care plans and ensure a holistic approach to resident care.
Medication Knowledge:
- Demonstrate a thorough understanding of medications, including dosages, side effects, and therapeutic effects, to provide safe and effective care.
- Educate residents about medication usage, potential side effects, and health management strategies.
Wellness Program:
- Maintain an assigned wellness list, monitoring residents' overall health and well-being.
- Contribute to the evaluation and updating of wellness programs to ensure they meet the evolving needs of residents.
Teamwork:
- Collaborate effectively with colleagues to ensure a cohesive and supportive team environment.
- Participate in problem-solving and decision-making processes to improve care quality and team efficiency.
