The North Central Health District’s Home Visiting Program is a warm, community-centered initiative designed to bring high-quality healthcare, education, and support directly to families in the rural areas of our district. Our team works closely with expectant and new parents, helping them navigate pregnancy, birth, and early childhood with confidence. By meeting families where they are, we build meaningful relationships, reduce barriers to care, and make a real difference in maternal and child health outcomes. Requirements

The primary function of this LPN role is to provide professional nursing care under general supervision in accordance with established protocols, policies, and procedures. Conduct outreach, support, and education for the target population and surrounding community to help reduce disparities in birth outcomes. Deliver community health and case management services to high-risk perinatal women, infants, and children, primarily through home visits. Educate patients and connect them with resources and agencies that can support their social and health needs. Accurately document observations and patient responses to care.


Primary Duties & Responsibilities


  • Focus on preventing illness, injury, and disability while promoting and maintaining the health of individuals and communities.

  • Provide care to populations with special needs during disasters and emergencies, including situations involving biological and chemical protocols.

  • Conduct home visits to identify, assess, and provide care coordination services for eligible perinatal patients and qualifying infants.

  • Coordinate program services with other providers and community organizations to ensure effective service delivery.

  • Screen clients and refers them to appropriate community resources and service providers.

  • Provide education, monitoring, and materials to support wellness, safety, and awareness of pregnancy and postpartum complications.

  • Monitor infants for growth and development, including feeding, weight, head circumference, and developmental milestones.

  • Develop and update care plans for individuals and families based on their needs and responses to treatment.

  • Provide referrals and connect clients to appropriate services when medical or environmental risks are identified.

  • Promote the program through community outreach and engagement activities.

  • Build and maintain partnerships with community organizations.

  • Deliver case management and nursing care in accordance with public health protocols.

  • Perform physical assessments, takes vital signs, conduct fetal monitoring, and completes basic lab screenings.

  • Provide breastfeeding education and counseling and connect clients to appropriate health and social services.

  • Maintain accurate and timely documentation of patient care, services provided, and patient outcomes.

  • Preform Additional duties as assigned.

This position is based at the North Central Health District’s Central Office in Macon, GA, and requires travel to provide home visits across Baldwin, Bibb, Hancock, Jones, Peach, Washington, and Wilkinson counties, with occasional visits to additional counties such as Crawford, Houston, Jasper, Monroe, Putnam, and Twiggs. This will change as the program expands. Candidates should reside within a reasonable commuting distance and be willing to travel to locations throughout our district as needed. Employees in this role are eligible to receive mileage compensation for work-related travel. This is an excellent opportunity to work directly with communities across the district and make a meaningful impact.


Qualifications

High School diploma/GED and completion of an LPN Program through an approved Georgia Nursing Board; or Vocational/Technical degree from an accredited college and current Georgia license to practice as a Licensed Practical Nurse and six months of nursing experience; or one (1) year of experience required at the lower-level Licensed Practical Nurse 1 (HCT030) or position equivalent. Must have a valid driver’s license and reliable transportation.


Preferred Qualifications

  • Bilingual in Spanish and English

  • Bachelor’s degree in Nursing (BSN)

  • Experience providing interventions for high-risk prenatal patients

  • Experience with high-risk newborns and infants

  • Home visitation experience

  • At least one year of labor and delivery experience

  • At least one year of experience working in a prenatal care clinic

  • At least one year of experience working with mothers and infants

The use of physical activity breaks during the workday is authorized to support this philosophy and help employees meet their wellness goals. A maximum of 30 minutes of physical activity may be allocated per workday, typically in the form of two 15-minute breaks or one 30-minute break.

DPH accepts educational credentials recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) as valid. DPH will contact educational institutions to verify degrees, diplomas, licensures, and other relevant credentials.
