In this role you will use your nursing knowledge and expertise to provide surveillance, consultation, guidance, audits and evaluation of health care providers to assure a level of quality care based upon compliance with Federal and State laws and rules.  This position will also provide training and expertise to health care professionals in an effort to educate them on the MDS Assessment standards to ensure compliance. 

Working as a Health Facility Evaluator, you will:

  • Evaluate and conduct audits on providers to ensure a level of compliance with State and Federal laws and rules and to audit the accuracy of coding MDS assessment forms and the resulting Case Mix classifications and to provide training to providers, as allowed, to prevent violations as related to applicable regulations and standards of practice.
  • Compile and record the information gathered to document conclusions reached and ascertain the degree of compliance with regulations.
  • Make periodic unannounced or announced follow-up site visits to ascertain compliance with correction orders previously issued or as an expanded pool audit is determined necessary.
  • Participate in continuing education program to develop and expand working knowledge in the health care field and maintain current licensure as a Registered Nurse.
  • Attend legal proceedings regarding correction orders, issuance of penalty assessments, so that all matters and concerns therein will be properly presented and clarified.         

This position will be based out of the Rochester office; however, the majority of work time will be spent onsite performing surveillance of facilities predominately located in the south central and southeastern region of Minnesota. 

Occasional travel outside of the base region is expected, to survey facilities located throughout the state of Minnesota.  When not onsite, remaining follow-up work is performed by teleworking from your home office.

The general hours of work are between 8:00 a.m. - 4:30 p.m., Monday- Friday, but candidates must be available for occasional weekend work and overnight travel.  The need for weekend, overnight, or travel outside of the base region is determined by inspection workload.  Reimbursement for authorized business expenses is provided according to bargaining unit guidelines.

Minimum Qualifications

Currently licensed in Minnesota as a Registered Nurse, without restriction

AND

Two (2) years of experience in a health care setting in positions that may include the following:  

  • Registered Nurse (RN)
  • Charge Nurse (must include at least two years as an RN charge nurse)
  • RN Supervisor
  • Director of Nursing
  • Health Care Administrator

A related degree in Nursing, Public Health, Health Care Administration, Psychology, or closely related medical field may substitute for experience as follows:

Master's = 18 months

Bachelor's = 12 months

Associate = 6 months

Additional requirements to be assessed during the interview process:

  • Communication skills that include verbal skills sufficient to interview staff, residents, clients and patients, listening skills to ascertain level of compliance in meeting resident or client needs; and written skills sufficient to document concise and accurate information and conclusions.
  • Critical thinking skills sufficient to collect and analyze information, identify areas of focus and make decisions based on evidence gathered.
  • Human relations skills to develop and maintain effective working relationships, inform providers of deficiencies and participate in legal proceedings.

Preferred Qualifications

  • Experience working in a federally certified care facility and with the federal informal dispute resolution process for federally certified nursing homes.  
  • Knowledge of federal and state laws and regulations pertaining to nursing and health services and the ability to interpret these regulations for implementation.  
  • Successful completion of the Federal Surveyor Minimum Qualification Test (SMQT). 
  • Experience conducting internal quality assurance activities. 
  • Experience performing surveys and making deficiency decisions related to nursing, medical and health care.  

Additional Requirements

  • This position requires successful completion of a background check and employment reference check.  
  • This position does not require a valid driver's license, but it does demand travel, which the candidate will be responsible for meeting. 
  • Successful completion of the Federal Surveyor Minimum Qualification Test (SMQT) after training provided by the MDH.
