Under the general supervision of the Children and Youth with Special Health Care Needs (CYSHCN) Manager in the Maternal and Child Health (MCH) Section, the CYSHCN Community Resource Coordinator performs advanced consultative contract services and technical work related to contract planning, development, and implementation of systems of care for CYSHCN and their families. This position manages and coordinates all contract details for all CYSHCN statewide community-based contractors and the program-directed contract development and management, contract work plan development and approval, and contract quality assurance and improvement activities. This position’s activities support essential public health services and efforts to achieve Title V Maternal and Child Health Block Grant performance measures related to Texas CYSHCN and their families. This position works to coordinate, develop, and carry out all CYSHCN contract Title V Block Grant Action Plan activities. This position independently performs associated contract planning and collaborative activities with other agency programs, organizations, and groups that address CYSHCN issues. This position works under limited supervision with considerable latitude for the use of initiative and independent judgment.

Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
Leads and manages comprehensive program contract oversight for the CYSHCN Systems Development Group contracted community initiatives with extensive authority for contract decisions regarding the details of contract development, planning (workplans with short and long-term goals), implementation, quality assurance and improvement activities, reporting, evaluation, networking, and coordination. Leads the development of contract request for proposals (RFPs) for program initiatives and CYSHCN application review and/or contract renewal processes. Negotiates contracts with selected community organizations and consults with internal stakeholders and regional staff throughout the process. Responsible for CYSHCN contract budget development and funding proposals. Reviews and evaluates CYSHCN contract service delivery methods, outputs, and activities; identifies contract resource gaps; and makes recommendations for contract improvement and funding based on review criteria. Schedules and performs CYSHCN contract quality assurance monitoring visits as needed. Responsible for the development of CYSHCN contract quality assurance monitoring materials, preparation of on-site visits or desk reviews, and development of evaluation reports. Leads the annual CYSHCN contract review of standards and the initial development of standards. Performs CYSHCN contractor’s programmatic risk assessments for assigned initiatives. (35%)
Plans and coordinate activities for specific MCH Section CYSHCN community-based initiatives contracts that support Title V mandates, promote progress toward the Title V CYSHCN performance measures conducted in collaboration with other Title V MCH staff, other DSHS staff (central office and regional), providers, and families. Establishes CYSHCN contract program guidelines, goals and objectives, procedures, standards, and timelines. Provides contracts consultation and technical assistance to providers and contractors and the DSHS regional staff. Identifies needed CYSHCN contracts areas of change and makes recommendations to evaluate and improve program contract activities. Works with stakeholders in assessing CYSHCN contract service needs and gaps and supports development and/or redesigning systems of care. Collaborate with program staff in contract planning, development, and implementation of new contract initiatives and the development of tools and instruments for collecting and analyzing contract data. Assists in reporting on contract activities for the CSHCN Systems Development Group. (30%)
Establishes effective CYSHCN contracts partnerships and collaborative workgroups with other state agencies, disability service/advocacy organizations, consumer and provider groups, funding entities, and internal DSHS partners to further awareness, needs, and issues of CYSHCN and their families and collaboratively plan contract activities to address Title V CSHCN performance measures. Represent the CYSHCN Systems Development Group at meetings, hearings, conferences, and seminars or on boards, panels, and committees. Promotes use of contract evidence-based practices. Seeks opportunities to integrate CYSHCN contract services for establishing medical homes, health care transition, and community inclusion for CYSHCN and their families. (15%)
Independently researches program policy, budget, and contract impact of services provided, systems changes, and grant proposals, including analysis of contractor data, and other state and federal programs, legislation, and strategic planning, tracks and reports on progress towards contract strategic planning objectives. Determines CYSHCN contract training needs, develops and may deliver contract educational/training tools, presentations, and curricula on issues pertaining to CYSHCN to providers (including contractors), families, and staff through various formats (15%).
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency’s obligations for disaster response and/or recovery, Continuity of Operations (COOP) activation, and/or serving on the State Medical Operations Center (SMOC) team. Such participation may require an alternate shift pattern assignment and/or location. (5%)
