Main Purpose of Job
The Child and Adolescent Service Coordinator/ Family Services Grant Monitor is responsible for coordinating services related to children, adolescents, young adults and families that are in need of behavioral health services including community based and out-of-home placements. This position ensures quality of the contract sub-vendors. This position also conducts grant monitoring of contracts and tracks funding. This position is eligible for a Hybrid Telework Schedule.
POSITION DUTIES
35% Child and Adolescent Service Coordination Link, refer, and follow up on consumers (children, adolescents, and young adults) and families for mental health and SUD services including family supports through community-based services to prevent inpatient hospitalization and/or out-of-home placements such as Residential Treatment Center, Therapeutic Group Home, and Therapeutic Foster Care. Support youth and families in navigating the Public Behavioral Health System by educating parents, caregivers, and community partners on available community-based resources and supportive services to ensure youth remain in the home/ community while receiving needed care. Monitor children, adolescents and young adults who receive multiple services and organize case conferences to address identified needs. Identify gaps in behavioral health services related to services for children, adolescents and young adults and participate in resource development. Coordinate community-based services when youth are returning to the community from a RTC, in-patient psychiatric unit, or Therapeutic Foster Care Placement. Serve as an entry point to the Transition Aged Youth (TAY) Grant. Referrals are screened and reviewed for eligibility and need. For the 12 TAY RRP Beds (once deemed appropriate), the referral is sent to the residential program by order of priority. Maintain a waitlist for appropriate referrals to the TAY RRP. When beds become available, share referrals from the waitlist. Work to increase awareness and education to families about the Targeted Case Management (TCM), Targeted Case Management Plus (TCM Plus) and 1915(i) services that are available to children, adolescents and their families in Howard County. When a youth qualifies for TCM or 1915(i) services, and the family is interested, facilitate connecting to these services. Work in collaboration with the Administrative Services Organization (ASO) to disseminate accessible TCM, TCM Plus and 1915(i) information to families and providers. Attend community outreach events to educate families, County partners, Local Care Teams and child-serving agencies on services available to youth and families. Complete ESCII and CASII Assessments on any youth that has been recommended for RTC Level of Care or Level 3 Care Coordination. Review and approve all Certificates of Needs (CON) documents for children/adolescents in which RTC level of care has been recommended. Review and approve all CONs for Level 3 Care Coordination. Review RRP-TAY CON requests. Submit approvals to the ASO. Address complaints for issues that arise related to child and adolescent consumers served by the public mental health system. Represent the Bureau of Behavioral Health on various teams and meetings. Be a representative on the Local Care Team (LCT). Prepare referral packets and make case presentations for Seriously Emotionally Disturbed (SED) referrals. Maintain regular contact and participate in treatment team meetings with clinical staff at Residential Treatment Centers (RTCs), in-patient psychiatric units. Participates in monthly all-staff meetings to stay current on agency activities and issues. Attend required meetings/trainings as well as meetings designated by the direct supervisor and/or the Bureau Director. 30% Quality Assurance Gather 1915(i) utilization and satisfaction data. Using surveys & other BHA-approved feedback mechanisms to gauge satisfaction of youth & families receiving services. Ensures that program data are accurate and submitted timely each month due by 3rd of each month and meets the Bureau and funding reporting requirements. Submit program relevant data monthly/quarterly/bi-annually to the Behavioral Health Administration (BHA)/Bureau Director by due dates. Submit monthly and/or quarterly behavioral health reports to Behavioral Health Administration, and/or the Local Behavioral Health Authority. Conducts site reviews 2 times a year with the program sub-vendors: TAY and crisis intervention team (CIT). Conduct site visits 1 time per year for youth target case management/care coordination. Completes grant monitoring paperwork by due dates (December and June), and implements corrective actions as needed. Monitors the quality and quantity of program services and outreach efforts in order to maintain or improve services. 30% Grant Monitoring of Contracts/Tracking of Funding Grant monitoring of contracts and funding tracking relevant to the mission of the Bureau with a total budget of approximately $210,000. The contact person with the grant monitoring program sub-vendors: TAY, crisis intervention team (CIT), and youth target case management/care coordination (TCM) and 1915i services to answer questions and provide technical assistance as needed. Address contract vendor questions, serve as the point of contact for the vendors, and provide technical assistance. Monitor contractors to ensure compliance with conditions of award, performance standards, expense limits, and service provisions. Conduct grant monitoring, site visits, and inspections bi-annually or as the monitoring schedule indicates. Submit audit documentation to the Behavioral Health Administration (BHA)/Bureau Director by due dates. Track total expenses utilized by contracted services in scope of work. Coordinate with Fiscal Services regarding referrals and invoicing as needed. Review/approve invoices associated with contracts in preparation for payment. Works with Fiscal Services/Contracts Unit to address any concerns with invoices, contracts, or purchase orders. Submit invoices to the Bureau Director for payment approval after ensuring the contract and purchase order have been executed and funding is sufficient. Track contract expiration and renewal dates to mitigate gaps in services. Work with the Contracts Unit to ensure contract creations, renewals and purchase orders are completed by due dates. Informs the Bureau Director of any contract creation/renewal/purchase order concerns, as soon as possible. Draft new contract worksheets with new vendors. Work with the Contracts Unit to ensure contracts are drafted within specification. Gain final contract approval from Bureau Director. Lead efforts working with vendors/Fiscal services to ensure 432 (budget) packets and 440s/440As (performance measures and expense reports are submitted on time and adequately completed). 5% Other duties as assigned Performs other related duties as assigned.
MINIMUM QUALIFICATIONS
Education: Graduation from an accredited high school or possession of a high school equivalency certificate. Experience: Two years of professional experience in assisting in the coordination of a health-related program(s) or referring people to governmental and private resources. Notes: 1. Candidates may substitute 30 college credit hours from an accredited college or university on a year-for-year basis for the required experience. 2. Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Health Care Administration classifications or Civil Affairs Specialist specialty codes in the health-related field of work on a year-for-year basis for the required experience and education.
