Come join our team! As a Care Management Coordinator with North Central Health District’s Children’s Medical Services program you will join a dedicated group of public health professionals working to protect the health and well-being of communities across the state by supporting vital public health initiatives. Requirements

Under the supervision of the Registered Nurse Supervisor, the Care Management Coordinator provides comprehensive case management and care coordination services to families enrolled in CMS. Responsibilities include developing and maintaining individualized care plans, managing and updating medical records, and supporting clients through the transition from pediatric to adult care to ensure continuity of services.


Primary Duties & Responsibilities


  • Schedule enrollment visits, annual reviews, and six-month updates in accordance with program policies and family/client requests.
  • Maintain and update Outlook calendar with appointments, home visits, meetings, and related activities.
  • Travel within North Central Health District’s 13-county service area to complete CMS home visits.
  • Document all client and family encounters in M&M, including communications with internal staff, external vendors, medical providers, and community partners.
  • Provide care coordination in accordance with program policies, including serving as a liaison between families and providers/vendors and assisting with transitions from pediatric to adult care per CMS guidelines.
  • Complete, review, edit, and scan documentation related to enrollments, annual reviews, six-month updates, and care transitions.
  • Ensure care notes and plans are reviewed and signed by licensed/credentialed personnel to maintain Medicaid compliance.
  • Collaborate with licensed providers actively involved in care planning and oversight.
  • Complete and document prior authorizations in accordance with CMS policy and family requests.
  • Request, review, scan, and upload physician notes and medical documentation; update client records in M&M.
  • Communicate with providers to clarify diagnoses, obtain prescription instructions, prior authorizations, and other information necessary to ensure continuity of care.
  • Research and provide internal and external resources to families as needed.
  • Attend meetings, health fairs, workshops, conferences, and outreach events as requested.
  • Assist with preparation and support for outreach activities and community events.
  • Submit travel documentation for supervisory approval.
  • Comply with HIPAA and all CMS, state, district, and Department of Public Health policies.
  • Provide administrative and/or clinical support in authorized shelters during public health emergencies as directed by Emergency Preparedness.
  • Support clinic operations as needed, including patient check-in, processing demographic and lab documentation, and responding to calls and voicemails.
  • Perform additional duties as assigned by physician staff or leadership.

Qualifications

Bachelor's degree in a related field from an accredited college or university and three (3) years of experience in social service delivery or one (1) year of experience as the lower level Social Worker, NonLicensed 1 (HCP120) or position equivalent.

The use of physical activity breaks during the workday is authorized to support this philosophy and help employees meet their wellness goals. A maximum of 30 minutes of physical activity may be allocated per workday, typically in the form of two 15-minute breaks or one 30-minute break.

DPH accepts educational credentials recognized by the Council for Higher Education Accreditation (CHEA) and/or the US Department of Education (DOE) as valid. DPH will contact educational institutions to verify degrees, diplomas, licensures, and other relevant credentials.
