Administrative Program Specialist A (CHW Supervisor)


The mission of the Louisiana Department of Health is to protect and promote health and to ensure access to medical, preventive, and rehabilitative services for all residents of the State of Louisiana.

Take a leadership role in supporting and managing Louisiana’s Community Health Worker (CHW) program across 9 regions and 60 parish health units. This position provides direct supervision, training, and programmatic oversight to administrative and contract CHWs, ensuring compliance with statewide policies and effective use of the CHW computer-based system.

By guiding records management, caseload follow-up, and program implementation, you will help improve the quality of life for Louisiana residents by addressing health-related social needs and the broader social determinants of health. This position is located in Lafayette Parish but also operates in Acadia, Evangeline, Iberia, St. Martin, St. Landry, and Vermilion Parishes

Requirements

Three years of experience in administrative services; OR

Six years of full-time work experience in any field; OR

A bachelor's degree.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.

  • Serves as the statewide lead for the CHW Program, providing policy guidance, system expertise, and programmatic consultation to CHW staff across all regions.
  • Supervises, trains, and orients CHW staff, ensuring proper use of the CHW computer-based system, implementation of program standards, and adherence to policies and procedures.
  • Acts as a “Super User” of the CHW system, configuring the system, defining clinical decision rules, troubleshooting IT issues, and providing frontline support to staff.
  • Manages program operations including onboarding, scheduling, approving time and travel expenses, coordinating supply requests, and maintaining standard operating procedures and program manuals.
  • Conducts patient needs assessments, provides support for health goal setting, social service referrals, follow-up calls, home visits, and assists clients with accessing medical, nutrition, and community resources.
  • Maintains accurate and detailed documentation of patient encounters, prepares reports, monitors caseload data, and identifies barriers to connecting clients with health and social services.
  • Coordinates with partners, IT staff, and the State Management Team to implement program improvements, professional development opportunities, and community-based initiatives to address social determinants of health.
