Requirements

  • Provides outstanding customer service
  • Maintains HIPAA standards related to Privacy and Security
  • Travels throughout the 14-county health district for home visits
  • Under supervision, facilitates and participates in the development, review, and evaluation of Individualized Family Service Plans for infants and toddlers with developmental delays/medical conditions
  • Assesses infants and toddlers to determine developmental needs
  • Assists families in gaining access to early intervention services
  • Coordinates multiple providers of services and works collaboratively with community agencies and providers
  • Coordinates developmental evaluations and assessments; monitors service provision for quality  
  • May participate in health fairs, serve on committees, and make presentations
  • Responds to Public Health Emergencies

Qualifications

Bachelor's degree in a related field from an accredited college or university and two (2) years of experience in social service delivery.. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date). Must be able to lift up to 45 lbs, stoop and bend.)

Preferred Qualifications

  • Public Health experience
  • Case Management Experience
  • Experience working with children with developmental disabilities
  • More than two years of experience in social service delivery

Highlight any experience related to the Preferred Qualifications. This position may require driving company vehicles (Valid driver’s license required) and working some evenings and weekends during special events such as health fairs and responding to disasters. Selected applicant must consent to a motor vehicle record assessment. Points should be less than 10 and/or not negatively impact insurance rates for the agency. Drug screen, criminal background check and immunization records may be required. If selected, male applicants between 18 and 26 years of age must present proof of Selective Service Registration.
