Requirements

  • Under the supervision of a nurse, provides home visiting services to eligible pregnant, postpartum, and inter-conceptual women and their babies in the counties of Colquitt, Decatur, Dougherty, Grady, Mitchell, Seminole, Terrell, Thomas, and Worth.
  • Performs appropriate clinical duties for the enrolled patients, such as obtaining health histories, vital signs, weights, heights, and other point-of-care testing. Makes assessments to determine needs and makes medical and other resource referrals as indicated. 
  • As a community health worker, provides outreach, support, and patient care in order to reduce disparities in birth outcomes.
  • May handle some administrative duties, which include scheduling appointments, maintaining medical records, documenting electronic medical records, etc.
  • May participate in health fairs, community events, and respond to disasters.
  • Provides outstanding customer service and maintains HIPAA standards related to Privacy and Security
  • Responds to Public Health Emergencies
  • Must have reliable transportation

Qualifications

Vocational/Technical degree in licensed practical nursing from an accredited college AND current Georgia license to practice as a Licensed Practical Nurse. (Please mail or bring a copy of College Transcript or Diploma to Interview—must have official transcript prior to start date). Physical Requirements include occasional lifting of up to 45 lbs. 

Preferred Qualifications

  • Previous employment with any state’s Department of Public Health or a County Board of Health
  • Home visitation experience
  • Experience working with pregnant women and with infants
  • One year of Labor & Delivery experience
  • Bilingual (Fluent in Spanish and English)


A State Merit Application for Employment must be received by this office no later than the deadline.  Highlight any experience related to the Preferred Qualifications.  This position may require driving company vehicles (Valid driver’s license required) and working some evenings and weekends during special events such as health fairs and responding to disasters.  Selected applicant must consent to a motor vehicle record assessment.  Points should be less than 10 and/or not negatively impact insurance rates for the agency.  Drug screen, criminal background check and immunization records may be required.  If selected, male applicants between 18 and 26 years of age must present proof of Selective Service Registration.  All information submitted is subject to verification.
