Job Description:  Under general supervision, provide Care Coordination in a specialized Public Health program, that serves Children and Youth with Special Medical Needs, per CMS policy and procedure manual.  
 
 All District 4 Public Health, County BOH employees are deemed essential employees in the event of an emergency. As such, you may be called on to participate in an event not limited to but including, staffing a shelter, a Specimen Point of Collection Site (SPOC) and/or Point of Dispensing site (POD). When you are called on to participate, you are expected and it will be a part of your job requirement to participate, whether the emergency occurs during or outside regular work hours.

Requirements

  • Conduct initial, annual, and other periodic medical and social assessments of clients with special needs and their families on the phone, at clinic or in the home within designated counties.  
  • Partner with the family and caregivers to assess a client’s condition and develop a plan of care. 
  • Learn about and interact with community partners, other public and private service agencies, and local/regional resources. 
  • Serve as an advocate and help overcome obstacles in order to connect clients with chronic illnesses to appropriate specialty care and community supports.
  • Educate clients and caregivers about the need to transition to adult special services
  • Take notes and provide assistance in our CMS orthopedic clinic as needed. 
  • Record patient care including all pertinent observations and information in client chart and EMR.
  • Learn how to take a referral, determine medical and financial eligibility for the CMS program, and refer to other PH programs. 
  • Understand and document appropriately for billing and for program reports. 
  • Prepare to respond to public health emergencies and respond as directed within your scope of practice. Incorporate strategies for interacting with persons from diverse backgrounds e.g. cultural, socioeconomic, educational, racial, gender, age, ethnic, sexual orientation, professional, religious affiliation, mental and physical capabilities.
  • This position requires travel to all 12 District Four counties. Travel reimbursement will be based on current State of Georgia Travel regulations.

Qualifications

Social Worker, Non-Licensed 1 - (Annual salary - $39,060.94) 

Master's degree in a related field from an accredited college or university AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Two years of experience in social service delivery. 

 

Social Worker, Non-Licensed 2 - (Annual salary - $42,467.04) 

Master's degree in a related field from an accredited college or university AND One year of experience AND eligibility to be licensed OR Bachelor's degree in a related field from an accredited college or university AND Three years of experience in social service delivery OR One year of experience at the lower level Social Worker, NonLicensed 1 (HCP120) or position equivalent.

PH Licensed Practical Nurse Level 3 - (Annual salary - $38,926.79) 

Vocational/Technical degree in licensed practical nursing from an accredited college AND Four years of LPN public health experience AND current Georgia license to practice as a Licensed Practical Nurse AND CPR certification.

Preferred Qualifications:  

  • Public Health experience.
  • Experience or interest in health and disabilities
  • Bilingual Spanish/English Speaking Language.
