Social Worker/Disease Intervention

Under the general supervision of the Program Manager of the Disease Intervention Specialist (DIS) Division, the Social Worker will provide Direct Services, Clinical Coordination, and Community Outreach throughout the Midlands Public Health Region.

Direct Services in support of Clients:
-Individual Client Services using the Anti-Retroviral Treatment and Access to Services (ARTAS) for the linkage to care for newly Human Immunodeficiency Virus (HIV) positive diagnosed clients and identified out of care clients. 

-Use appropriate social work skills and techniques to plan, document, implement, and coordinate the delivery of risk-reduction programming and referrals in the Region. 

-Provide Comprehensive Risk Counseling and Services (CRCS)  within the Region, assigned sites, and activities as requested for recalcitrant clients.

Clinical Coordination and Documentation as required:
-Work with other the agency's employees and with community agencies to coordinate care for individual clients. 

-Document professional social work services on the appropriate forms and following appropriate guidelines. 

-Maintain documentation in clinical record within appropriate time frame to submit reports timely.

Community Services and or Leadership:
-Establish maintain community relationships, resources, and appropriate referral services for clients and program development. 

-Participate in activities in the agency and community to enhance efforts to reduce the impact of Sexual Transmitted Diseases (STD)s, (HIV), Acquired Immunodeficiency Syndrome (AIDS/) and Viral Hepatitis. Participate as part of observed National (HIV) and (AIDS) Awareness Days.

Continued Quality Improvement and Staff Development:
-Participate in quality improvement activities and trainings to improve effectiveness of social work and program activities. 

-Maintain licensed as a professional Social Worker.

Emergency Preparedness

-Participate in emergency situations as a behavioral health expert as required by the agency, and other agency requested activities.  

The headquarters for the position are negotiable within the counties of the Midlands Public Health Region ( Aiken, Barnwell, Chester, Edgefield, Fairfield, Kershaw, Lancaster, Lexington, Newberry, Richland, Rock Hill Health Department (York), Saluda, York County. 

Requirements

A bachelor's degree in social work from a program accredited by a nationally recognized accrediting body for social work programs, or from a social work program whose standards are at least equivalent to the minimum standards required by the nationally recognized accrediting body approved by the Board.


Institutions of Higher Learning must be recognized by the Council for Higher Education Accreditation.

Additional Agency Requirements:

Masters Degree in Social Work from a school of social work accredited by the Council on Social Work Education.· Licensed as an Licensed Master Social Worker (LMSW) or Licensed Independent Social Worker ( LISW ) in good standing by the SC Board of Social Work Examiners, or SC Licensed Labor Licensing and Regulation (SC LLR).


Qualifications

Experienced in assisting with access to care for persons living with HIV and having provide services.   This position is part of the South Carolina Department of Health and Environmental Control’s Disease Intervention. The work performed supports DHEC’s mission to improve the quality of life for all South Carolinians by protecting and promoting the health of the public and the environment. Effective July 1, 2024, this position will transfer to the South Carolina Department of Public Health created by Senate Bill 399 which provides for the creation of the Department of Public Health to assume the public health-related functions of DHEC. 


Driving Record: If this position requires the applicant to possess a valid driver’s license to operate a state vehicle or personal vehicle, any applicant being considered in the final stages of selection for the position will be required to provide a certified copy of a 10-year driving record.
