Utilizes the nursing process, assesses the health of clients, and assists in developing and follows care plans. Documents care/services per agency policies/procedures. Identifies client/family needs and priorities for children seeking or receiving CYSHCN services and formulates appropriate plans of care. Determines eligibility for financial assistance according to CYSHCN policy manual; authorizes services for eligible persons as needed. Serves as care coordinator for assigned caseload and implements plan of care via visits or contacts with client/family.

Provides nursing care, and participates in providing patient education, guidance and care coordination for client /families within the CYSHCN population. Coordinates case finding and services with other disciplines as needed. Makes referrals for medical, socioeconomic, mental and nutritional needs as identified and appropriate. Assists with screening and physical assessment of campers on check-in days at Camp Burnt Gin as assigned by supervisor.

Participates in Continuous Quality Improvement. Collects and reports on characteristics of assigned caseload, services provided and outcomes as requested by supervisor. Participates in program audits as requested. Participates in development and review of CYSHCN policies and procedures related to the delivery of CYSHCN as requested. Participates in staff meetings. Identifies training needs to supervisor. Assists with the training and mentoring of new CYSHCN staff.

Maintains professional growth. Completes CYSHCN program and agency required trainings. Represents office or region on committees, workgroups and community events as assigned. Completes continuing education of at least 20 contact hours per year.

Requirements

State Minimum Requirements: Licensure as a registered nurse by the State Board of Nursing for South Carolina. Graduation from an accredited school of nursing.

Agency Additional Requirements: An associate degree or diploma in Nursing and current licensure as a Registered Nurse by the State Board of Nursing for South Carolina or a compact state. Two (2) years of nursing experience.

DHEC employees are required to participate in disaster preparedness during hazardous weather or declaration of an emergency. This involves being subject to duty and or call on a 24-hour basis.
You must possess a valid driver's license and have access to a reliable transportation.
This position may require working early (before 8:30 am), evening (after 5:00 pm) and/or Saturday's.
Ability to lift 30 pounds and stand for long periods of time, if required.

Must be able to respond timely and appropriately to frequently changing conditions and to make recommendations concerning procedural improvement to office/maintenance operations.
Must be able to drive to outlying counties and to non-traditional settings.
May be required to travel overnight.
 
IMMUNIZATION REQUIREMENTS
: All new employees who are healthcare providers with job-specific exposure risk are required to provide documentation of immunity or be immunized against measles, mumps, rubella, pertussis, varicella, and Hepatitis B prior to beginning employment.

Institutions of higher learning must be recognized by the Council for Higher Education Accreditation.


CURRENT NURSING STUDENTS WILL BE CONSIDERED:
Current nursing students or recent graduates will be considered prior to graduation and/or obtaining their nursing license should they meet all position requirements and provide proof of the obtained degree, transcripts, and nursing license within six (6) months of applying. Otherwise the offer will be rescinded.

The work performed supports DHEC’s mission to improve the quality of life for all South Carolinians by protecting and promoting the health of the public and the environment. Effective July 1, 2024, this position will transfer to the South Carolina Department of Public Health created by Senate Bill 399 which provides for the creation of the Department of Public Health to assume the public health-related functions of DHEC.
