Requirements

Purpose of the Position: Supervises and provides oversight of Children First/1st Care Program and case managers. Performs comprehensive assessments or clinic assessments (in home and or via telehealth) of client’s health care needs and developmental screenings within guidelines mandated by the Children 1st/1st Care programs.   Develops comprehensive plans of care for 1st Care clients.  Provides case management to patients/families consistent with care plan and BOH Policies and Procedures.  Provides health education instruction and/or classes to individuals, families and others for the promotion and maintenance of health and normal development.   Screens and refers clients to other community service providers as the need indicates. Assist with program evaluation and special projects, participates as a member of committees such Douglas Core or Child Death Review Committee. This position works to promote and protect the health and safety of the residents of Cobb and Douglas Counties.
 
Essential Functions (include, but are not limited to): 
  1. Direct supervisor of assigned case management staff which includes but is not limited to recruiting, onboarding, goal setting, performance management, etc.
  2. Perform comprehensive assessment of clients' health care needs within guidelines mandated by the Children 1st/ 1st Care program. 
  3. Implement and provide oversight for 1st Care Plans of Support
 

Qualifications

Minimum QualificationsBachelor’s degree in Nursing from an accredited college or university AND one year of nursing experience AND current Georgia license to practice as a Registered Professional Nurse AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND Two years of nursing experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification
 
Preferred QualificationsBilingual English/SpanishExperience with case management, children and adolescents with chronic medical conditions and special healthcare needs.
 
Work Conditions & Physical DemandsAbility to carry objects with weight not to exceed 50 lbs., maneuver stairs without difficulty due to home visitations.  
 
Salary Information: $65,243.00 Annually / Pay grade M
