As a Registered Nurse 2 within the West Central Health District, you will join a dedicated group of public health professionals that help support the overall mission and vision of protecting lives of Georgian residents in a variety of ways.

Requirements

You will support the Department by utilizing your expertise to fulfill the following responsibilities:

  • Travel throughout the Chattahoochee, Clay, Harris, Muscogee, Quitman, Randolph, Stewart, and Talbot county health departments to provide case management and nursing assessments to families that qualify for the 1st Care/CMS (Children’s Medical Services) program
  • Perform in-home comprehensive assessments of client’s health care needs & developmental screenings within the guidelines
  • Develop comprehensive plans of care for clients and provides case management to patients/families consistent with the plan of care, program deliverables, and District policies and procedures
  • Provide developmental screenings, patient education & counseling, advise caregivers and make necessary referrals for other social services
  • Serve as liaison between families & vendors and, assist with transitioning families from pediatric to adult health care, and participate in orthopedic, genetic & pediatric clinics.

Qualifications

MINIMUM QUALIFICATIONS: Bachelor’s degree in Nursing from an accredited college or university AND current Georgia Registered Professional Nurse license AND CPR certification OR Associate’s degree in nursing from an accredited college or university AND One year of PHN experience AND completion of a PHN course within one year of hire AND current Georgia Registered Professional Nurse license AND CPR certification. *A copy of college transcripts is required.

PREFERRED QUALIFICATIONS: Completion of a Bachelor’s degree in Nursing and one (1) year of experience as a Registered Professional Nurse.

All West Central Health District 7 employees are deemed essential workers in the event of an emergency. As such, you may be called on to participate in an event including but not limited to: staffing a shelter, working at a Specimen Point of Collection Site (SPOC) and/or working at a Point of Dispensing site (POD). If you are called on to participate, you are required to report to the designated site as a condition of employment, whether the emergency occurs during or outside regular work hours.
