Nurse 3 - Health - BPH Kanawha Co.

Incumbent provides medical care coordination to children with complex health problems from birth through 20 years of age and nursing services for Children with Special Health Care Needs (CSHCN), community and nutrition clinics as set form with the policies and procedures of the CSHCN Program.  

The Incumbent is responsible for medical care coordination and nursing services for clients within a specific geographical area of West Virginia.  Link clients and families to health services which assures access to comprehensive quality systems of care.  Engage in medical assessment and the care planning process as an active member of the medical home multidisciplinary team.  The Incumbent provides continuous reassessment of the medical needs of clients through Home/face-to-face visits, frequent review of all client records and specialized clinic visits.  Provides quality assurance services by ensuring care/care plan meets the current Bright Futures recommendations of standards of care.  

Incumbent participates in steering committees, task forces, committee meetings, etc. to represent CSHCN as assigned by supervisor.  Maintains communication with all staff involved in the care of the client.  Documents all contacts with clients/families and any other members of the medical home and ensures documents are forwarded to the state file as outlined in policy and procedure.  Participates in professional and departmental/program training to maintain nursing license and improve job performance skills.  Work with durable medical equipment and medical foods vendors.  Partner with families to assure they are informed about insurance coverage and benefit eligibility.  Improve understanding of CSHCN by local community medical providers.  Submit all required monthly reports to the State office as required by policy and supervisor.  This position may require Statewide travel.

Qualifications

Training: Baccalaureate degree in nursing from an accredited four-year college or university and two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program and three years of experience as a registered professional nurse.
OR
Professional certification from a recognized national nursing certifying organization as a nurse or nurse practitioner in the area of assignment plus two years of full-time or equivalent part-time paid experience as a registered professional nurse.
OR
Successful completion of an associate degree in nursing from an accredited college or university or a diploma nursing program; plus baccalaureate degree in the health sciences or behavioral sciences and two years of full-time or equivalent part-time paid experience in nursing.
OR
Master's degree in nursing from an accredited four-year college or university.
 
Special Requirement: Current West Virginia license or temporary permit to practice as a registered professional nurse.
