Licensed Practical Nurse

Woodward Resource Center (WRC) is seeking Licensed Practical Nurses to join our team. These are full-time opportunities with starting pay dependent upon experience with a possible shift differential of $0.90 per hour for second shift and $1.00 per hour for third shift.  WRC offers a strong opportunity for advancement, growth, and development.

The mission of Woodward Resource Center is to prepare and support individuals to live in the community of their choice. Discharge planning begins at admission.
As part of the WRC team: 
 

  • Assess and document health care problems for individuals according to policy and procedures.  This includes, but is not limited to completing Incident Reports, neurological checks, PNM nursing assessments, vital sign monitoring, and illness/professional contact related event logs.  Event log entries will be documented using SOAP (Subjective Objective Assessment Plan) method of charting.  The health care provider will be contacted per protocol or health care need.
  • Perform preventative health care practice which includes, but is not limited to: glucose checks, immunizations, routine scans and x-rays, and process routine lab orders.
  • Work with the RN and the IDT to provide appropriate nursing care and supports when a health care need is identified.
  • Prepare for and participate in all annual ISP meetings, Monthly Integrated Reviews, and Quarterly Psychiatric Treatment Reviews for assigned units when the RN is absent.
  • Provide assessment, and monitoring of self-administration of medication and medication administration procedures. Assist in completing training on all new and transfer employees on Health and Safety Support Plans. Participate in peer review as assigned unless otherwise excused, by giving constructive feedback/ideas on areas of improvement.
  • Ensure new monthly Medication Administration Records are complete and accurate. All nursing medications, treatments, and procedures will be performed according to policy and procedures and will be documented by the end of the shift. The LPN will be responsible for performing weekly checks on the MARs and Treatment records of their assignments and addressing any discrepancies according to policy and procedure.
  • Attend and participate in mini staffing or mini reviews, department meetings, and weekly team communication meetings unless otherwise excused.
  • Participate fully in all code blue drills. Complete Health Care Provider CPR every 2 years. 
  • Use universal precautions, procedures, and equipment 100% of the time for all anticipated contacts with blood and body fluids.
  • Other duties as assigned.

Qualifications

Licensure as a Licensed Practical Nurse by the Iowa Board of Nursing or licensure as a Licensed Practical Nurse in accordance with the Nurse Licensure Compact Administrator's Agreement of the National Council of State Boards of Nursing.
