Licensed Clinical Social Worker

THE JACKSON COUNTY DEPARTMENT OF PUBLIC HEALTH is accepting applications for a contract, part-time (up to 4 hours per week) Licensed Clinical Social Worker (LCSW) for our Maternal Health Clinic. The LCSW will provide counseling services to pregnant and postpartum patients, and coordinate the plan of care with the patients’ other providers. The LCSW will be responsible for assessing clients’ social needs through 1:1 assessments, intervention, and brief counseling to create care plans to address those needs appropriately through external referrals and in-house services as indicated by Maternal Health History in combination with validated screening tools. The LCSW will also manage external referrals to community-based organizations, health professionals, and other community support networks with the support of our Multidisciplinary Team. The LCSW will maintain complete confidentiality of all patient interaction and information.

Fully licensed LCSW to practice in North Carolina, required.
