42 CFR 483.430 (a) (1), a Qualified Intellectual Disabilities Professional is one of the following:
-A licensed psychologist (Master's prepared),
-A licensed physician (MD or DO),
-A social worker (Bachelor's or Master's prepared),
-An occupational therapist,
-A physical therapist,
-A speech pathologist or audiologist,
-A Registered Nurse,
-Professional recreation staff (an individual with a Bachelor's in a recreation specialty (art, music, physical education)
-An individual with a Bachelor's in a human services field (sociology, special education, rehabilitation, counseling, psychology).

What you will be doing:
In this role, you will serve as a Care Coordinator/Health Program Manager 2 for individuals receiving support through Home and community-based Medicaid waivers or out-of-state in Intermediate Care Facilities for Individuals with Intellectual Disability (ICF/IID). This position is responsible for person-centered planning and coordination, developing and submitting support plans (SP), transition planning, and evaluating service effectiveness. Applicants are expected to follow overarching program policies and rules independently.

In this role, you will:
  • Use a person-centered planning process to design a support plan for recipients with diverse needs.
  • Make regular in-person contacts as prescribed in the current regulations with all HCB waiver recipients assigned and regular contact with applicants/recipients, families, care providers, other agencies, and stakeholders. Additionally, you may be required to contact recipients in settings outside of Alaska.
  • Provide consultation to ensure compliance with SDS service principles, program standards, regulations, and statutes, as well as to ensure quality of care. 
  • Represent professional decisions throughout the Fair Hearing appeal process (7 AAC 49) as necessary. 
  • Gather, maintain, and assist with analyzing, interpreting, and reporting programmatic data to plan or reconfigure systems to improve service efficiency and quality, including providing technical assistance and conducting home and/or site visits as directed. 
  • Be eligible to be a certified Care Coordinator in compliance with the Care Coordination Conditions of Participation under 7 AAC 130.240.

The working environment you can expect:
Team members and supervisors are located in Anchorage at 1835 Bragaw Street, which is convenient to shops, restaurants, and trails. This position is scheduled for a 7.5-hour workday, but start and stop times can be flexible between 7 a.m. and 5:30 p.m.

Requirements

Any combination of education and/or experience that provides the applicant with the competencies in

 Equivalent to those typically gained by:
A Bachelor's degree from an accredited college in biological, health or behavioral science; health practice; education; public, healthcare, or business administration; or a closely related field;
AND/OR
Progressively responsible professional experience performing health program planning, development, coordination, evaluation, or implementation; providing technical health care assistance and consultation; conducting health care utilization or quality assurance examinations; and/or delivering health care.
 
Special Note: Agencies employing Health Program Managers are responsible for administering a wide range of medical, behavioral, and social health insurance programs for children and adults. Positions typically focus on a particular health area. Where licensure is required, the job vacancy announcement will identify the type of license necessary to apply for that specific position. The essential functions of some positions may involve exposure to infectious disease and/or biohazards. Travel may be required.
