Position Overview:
Under the supervision of the Outreach Program Supervisor, the Outreach Team Lead position will be responsible for developing and maintaining partnerships and creating/organizing outreach activities to educate, promote, and connect community members to all HCPH programs/services specifically on behalf of ACCESS and Healthy Start. The Outreach Team Lead is responsible for overseeing and supervising a team of Community Health Workers and providing guidance for their day-to-day tasks and professional growth.

Outreach Team Leads are direct liaisons for HCPH programs in the development, implementation, and updating of outreach strategies created to support all HCPH programs.

Duties & Responsibilities:
  • Oversee, manage, and train a team of Community Health Workers and provide day-to-day guidance.
  • Provide leadership to support the outreach team in coordination of program strategies, objectives, outcomes, and trainings.
  • Utilize a data-driven approach to research and find partners/locations in vulnerable or high-risk areas to deploy personnel for the distribution of information, resources, and awareness of HCPH programs throughout Harris County.
  • Track county-wide statistics, such as Social Vulnerability Index and indicators specific to programs, with the goal of mitigating disparities in priority populations and promoting HCPH programs.
  • Lead strategy development, implementation, data collection, reporting, and evaluation of outreach efforts for HCPH program(s) to demonstrate impact of increased utilization of HCPH programs.
  • Create and provide trainings/workshops/presentations for staff or community members/partner organizations.
  • Actively participate in outreach activities that strengthen partnerships and educate the community on HCPH programs/services, external resources, and relevant public health topics.

Position Type and Typical Hours of Work:

  • This is a full-time position.
  • Days and hours of work are typically Monday through Friday, from 8am to 5 pm.
  • Other days and hours as necessary to meet needs of team.
Work Environment:
  • This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machine.
Physical Demands:
  • This is a largely stationary role, however does require field work and many duties would require the ability to lift up to 20 pounds and bending and standing for periods at a time. This position requires driving of county vehicles on a week-to-week basis.
